Management of Elevated INR in Patients on Warfarin

Immediate Management of Elevated INR

  • An INR of 6.7 represents a medical emergency with exponentially increased bleeding risk, and the risk of major bleeding rises steeply above INR 6.0, according to the European Heart Journal and Circulation 1, 2, 3
  • For patients with prosthetic heart valves, do not use intravenous vitamin K, as it creates a risk of rapid overcorrection leading to valve thrombosis, and instead, admit to hospital, stop warfarin, and allow INR to fall gradually with serial monitoring, as recommended by the European Heart Journal 1
  • For patients without prosthetic valves, stop warfarin immediately, as recommended by Circulation 2, 3
  • Prothrombin complex concentrate is the treatment of choice for life-threatening bleeding, especially intracranial hemorrhage, according to the European Heart Journal 1
  • Low-dose IV vitamin K (1 mg) may be added if bleeding continues, recognizing the short half-life of Factor VII (6 hours), as recommended by the European Heart Journal 1

Reversal Strategy and Monitoring

  • The American Geriatrics Society recommends assessing antibiotic necessity and considering alternative non-interacting antibiotics, and if Augmentin is essential, proceeding only after INR is controlled 4
  • Check INR 2-3 days after starting Augmentin, then again at day 5-7, and continue monitoring every 3-5 days throughout antibiotic course and for 1 week after completion, as recommended by the American Geriatrics Society 4

Special Populations

  • Elderly patients (>75 years) have increased pharmacodynamic response to warfarin and require more frequent monitoring during antibiotic therapy, according to the American Geriatrics Society 4
  • The European Heart Journal recommends that patients with mechanical heart valves should not use IV vitamin K unless life-threatening bleeding is present 1
  • Never use high-dose vitamin K, as it creates a hypercoagulable state and warfarin resistance, according to Circulation 2, 3
