Management of Metastatic Colorectal Cancer

Diagnostic Priorities

  • The European Society for Medical Oncology recommends obtaining MRI of the spine immediately to characterize spinal lesions and determine if the patient has isolated liver-only metastases or multi-site metastatic disease 1, 2
  • The American Joint Committee on Cancer recommends assessing liver metastases resectability, with key determinants being sufficient remnant liver, technical feasibility, and tumor biology 2, 4

Management Algorithm

  • The National Comprehensive Cancer Network recommends a perioperative chemotherapy approach for liver-only disease with resectable liver metastases, including 3 months of preoperative FOLFOX, colectomy with en bloc lymph node resection, liver resection, and 3 months of postoperative FOLFOX 2, 4, 7
  • The European Society for Medical Oncology recommends intensive induction chemotherapy to achieve maximal tumor shrinkage for borderline resectable or unresectable liver metastases, with first-line regimen options based on patient fitness 2, 3
  • The American Society of Clinical Oncology recommends close monitoring with imaging every 2 months and multidisciplinary team review to assess conversion to resectability 2, 5

Systemic Chemotherapy Selection

  • The European Society for Medical Oncology recommends treatment intensity matching patient's biological age, performance status, and comorbidities, with options including intensive doublet or triplet chemotherapy with biologics for Group 2 patients 3, 8
  • The National Comprehensive Cancer Network recommends less intensive approaches, such as sequential single-agent therapy or capecitabine monotherapy, for Group 3 patients 3

Special Considerations

  • The International Society of Geriatric Oncology recommends comprehensive geriatric assessment for elderly patients, including assessment for frailty, comorbidity, and functional dependency 8
  • The American Geriatrics Society recommends involving geriatric medicine if significant comorbidities are identified 8

Critical Pitfalls to Avoid

  • The European Society for Medical Oncology recommends avoiding complete radiographic disappearance of liver metastases before resection, as this makes anatomical resection difficult and increases recurrence risk 2, 5
  • The National Comprehensive Cancer Network recommends monitoring patients on bolus 5-FU/leucovorin/irinotecan carefully due to increased 60-day mortality from gastrointestinal toxicity 7, 6
