Scoliosis Management Guidelines

Referral and Monitoring Criteria

  • The American College of Radiology recommends referral to an orthopedic specialist for skeletally immature patients with curves exceeding 20 degrees, as progression likelihood exceeds 70% 1, 2
  • Early-onset scoliosis requires both radiographic and MRI evaluation to exclude congenital anomalies and neural axis abnormalities, according to the American College of Radiology 1
  • Patients with curves between 20-26 degrees may be referred for bracing consideration, while those with curves between 26-45 degrees should undergo bracing and specialist management 1, 3
  • Surgical intervention should be considered for curves exceeding 40-50 degrees, particularly in patients with remaining growth potential, as recommended by the American College of Radiology 1, 3

Red Flags for Immediate Referral

  • The American College of Radiology recommends immediate referral for patients with atypical curve patterns, such as left thoracic curves, which may indicate neural abnormalities 1
  • Rapid curve progression (>1 degree per month) is a red flag for immediate referral, according to the American College of Radiology 2
  • Short segment curves (4-6 vertebral levels only) and absence of apical segment lordosis (hyperkyphosis at curve apex) are also indicators for immediate referral, as recommended by the American College of Radiology 2
  • Male patients with significant curves should be referred immediately, as idiopathic scoliosis is 10:1 female predominant for large curves, according to the American College of Radiology 2

Monitoring Schedule

  • The American College of Radiology recommends repeat radiographs every 12 months for patients with Risser stages 0-3 and curves <20 degrees, and every 18 months for those with Risser stages 4-5 1, 2

Special Populations

  • For patients with Duchenne Muscular Dystrophy (DMD) who develop scoliosis, referral is recommended when curves reach 30 degrees for surgical planning, according to the American Journal of Respiratory and Critical Care Medicine 4, 5
  • Surgery is typically scheduled when the Cobb angle is between 30-50 degrees, and timing is critical to intervene while lung function is satisfactory and before severe cardiomyopathy develops, as recommended by the American Journal of Respiratory and Critical Care Medicine 4, 5
