Alternative Antibiotics for Total Knee Arthroplasty Infection in Patients Allergic to Cephalexin

Introduction to Alternative Antibiotics

  • The American College of Surgeons and the Infectious Diseases Society of America recommend using clindamycin 900 mg IV as the primary alternative for perioperative prophylaxis in patients with cephalexin allergy and total knee arthroplasty infection, or vancomycin 30 mg/kg IV over 120 minutes for treatment of established infection, with pathogen-specific therapy guided by culture results 1, 2

Surgical Prophylaxis

  • For surgical prophylaxis, the primary alternative to cephalexin is clindamycin 900 mg IV slow infusion, with a single dose limited to the operative period (maximum 24 hours) 2
  • Vancomycin 30 mg/kg IV over 120 minutes is a secondary alternative, with infusion required to end at the latest at the beginning of intervention, ideally 30 minutes before incision 2

Treatment of Established Prosthetic Joint Infection

  • For methicillin-susceptible staphylococci, vancomycin is the primary alternative, while daptomycin and linezolid are alternative options 1, 4
  • For methicillin-resistant staphylococci, vancomycin is the first-line therapy, with daptomycin and linezolid as alternative options 4

Oral Suppression Therapy

  • After initial IV therapy, transition to oral antibiotics with rifampin for biofilm activity, with co-trimoxazole, minocycline, or doxycycline as recommended alternatives 1, 4
  • The duration of therapy is 6 months total for TKA infections, including 2-6 weeks IV 1, 4

Critical Monitoring and Safety Considerations

  • Monitor for vancomycin toxicity, including leukopenia, ototoxicity, and nephrotoxicity, particularly in patients with renal impairment 4
  • Monitor for linezolid toxicity, including cytopenias, peripheral neuropathy, optic neuritis, and risk of serotonin syndrome with concurrent SSRIs or MAOIs 4

Common Pitfalls to Avoid

  • Avoid timing errors with vancomycin prophylaxis, as the 120-minute infusion requirement means vancomycin cannot be given "on call to OR" 2
  • Always combine rifampin with a companion drug due to rapid resistance development 1, 4
  • Ensure adequate treatment duration, as TKA infections require 6 months total therapy 1, 4
