Enalapril Management in CKD G3a Patients (eGFR ≈ 57 mL/min/1.73 m²)

Continuation and Dosing

  • Enalapril should be continued at the current dose and renal function re‑checked within 2–4 weeks, because patients with CKD G3a still derive hypertension control and kidney‑protective benefits from ACE‑inhibition. 1
  • ACE‑inhibitor therapy remains appropriate even when eGFR falls below 30 mL/min/1.73 m²; an eGFR of 57 mL/min/1.73 m² is well within the safe continuation range. 2
  • The usual enalapril dose range (5–40 mg daily) is acceptable for creatinine clearance > 30 mL/min; no dose reduction is required for the patient described. 3

Monitoring Parameters

  • Serum creatinine, serum potassium, and blood pressure should be measured within 2–4 weeks after any dose change or when therapy is newly initiated. 4
  • Enalapril should be maintained unless serum creatinine rises > 30 % within 4 weeks of initiation or dose escalation. 1

Indications for Dose Reduction or Discontinuation

  • Reduce or stop enalapril only if symptomatic hypotension persists despite appropriate management. 5
  • Reduce or stop enalapril only if hyperkalemia remains uncontrolled after applying potassium‑lowering strategies. 5
  • Reduce or stop enalapril if serum creatinine increases > 30 % within 4 weeks of starting or increasing the dose. 2
  • Reduce or stop enalapril if eGFR falls below 15 mL/min/1.73 m² accompanied by uremic symptoms. 2

Optimizing Kidney Protection

  • Measure urine albumin‑to‑creatinine ratio (ACR) to stratify risk and guide adjunctive therapy. 3
  • If ACR ≥ 200 mg/g (≥ 20 mg/mmol), add an SGLT2 inhibitor (KDIGO 1A recommendation) for additional renal and cardiovascular protection. 1
  • If ACR 30–200 mg/g, consider adding an SGLT2 inhibitor (KDIGO 2B recommendation for eGFR 20–45 mL/min/1.73 m²; reasonable at eGFR 57). 2
  • Titrate enalapril to the highest tolerated dose (up to 40 mg daily) to achieve the renoprotective effect demonstrated in clinical trials. 4

Hyperkalemia Management

  • Hyperkalemia should be addressed with potassium‑lowering measures rather than discontinuing enalapril. 1
  • Implement dietary potassium restriction when hyperkalemia develops. 2
  • Monitor serum potassium regularly, especially within 2–4 weeks after any dose adjustment. 3

Common Pitfalls to Avoid

  • Do not discontinue enalapril prematurely for modest creatinine rises < 30 % or expected eGFR declines; these changes are often benign and associated with improved outcomes. 1
  • Do not combine enalapril with an ARB or a direct renin inhibitor, as dual RAS blockade increases adverse events without added benefit. 5
