Airway Pressure Management in APRV Settings

Introduction to APRV Settings

  • The mean airway pressure for APRV settings is approximately 28-29 cmH₂O, calculated using a specific formula, and actual measured values may be slightly higher due to spontaneous breathing efforts and pressure spikes during the release phase 1

Critical Safety Concerns

  • The American Thoracic Society strongly recommends maintaining plateau pressures ≤30 cmH₂O in ARDS patients to prevent alveolar overdistension and barotrauma, and current settings with Phigh of 30 cmH₂O and PIP of 34 cmH₂O may place the patient at significant risk for ventilator-induced lung injury 2, 3

Understanding APRV Parameters

  • Phigh of 30 cmH₂O is at the upper safety limit for plateau pressure, and reducing it to 26-28 cmH₂O may create a safety margin below the 30 cmH₂O limit 2, 3
  • PIP of 34 cmH₂O is concerning and should be monitored to remain ≤30 cmH₂O after adjustments 3

Spontaneous Tidal Volume Concern

  • The spontaneous tidal volume of 686 mL is excessive, likely representing approximately 10-12 mL/kg predicted body weight, and significantly exceeding the lung-protective target of 6-8 mL/kg, increasing the risk of ventilator-induced lung injury 2, 4, 5

Immediate Recommendations for Optimization

  • Reduce Phigh to 26-28 cmH₂O to create a safety margin below the 30 cmH₂O plateau pressure limit 2, 3
  • Increase Plow to 5-8 cmH₂O to maintain alveolar recruitment during the release phase and prevent cyclic collapse 2
  • Monitor PIP to remain ≤30 cmH₂O after adjustments 3
  • Consider sedation adjustment to reduce excessive spontaneous breathing effort generating high tidal volumes 2
  • Evaluate for pain or anxiety driving high respiratory effort, and consider neuromuscular blockade for 24-48 hours if spontaneous volumes remain >8 mL/kg despite optimization 2, 4

Monitoring Parameters During APRV

  • Arterial blood gases should be assessed every 2-4 hours initially to evaluate oxygenation and ventilation adequacy 4
  • Continuous plateau pressure monitoring is essential to ensure it remains ≤30 cmH₂O 3
  • Mean airway pressure trending on ventilator display should be monitored, targeting 25-28 cmH₂O for this patient 1
  • Hemodynamic monitoring is crucial as elevated mean airway pressures can impair venous return and cardiac output 1

Alternative Ventilation Strategy Consideration

  • Given the marginal safety profile of current settings, consider transitioning to conventional lung-protective ventilation with volume control, tidal volume 6 mL/kg predicted body weight, PEEP 10-15 cmH₂O, and plateau pressure ≤30 cmH₂O 2, 4
