Management of Acute Bronchospasm
Treatment Approach
- For moderate to severe acute bronchospasm, the American College of Chest Physicians recommends using Duoneb (ipratropium plus albuterol) rather than albuterol alone, as ipratropium provides additive bronchodilation particularly during the first few hours of an acute exacerbation 1, 2
- The National Asthma Education and Prevention Program (NAEPP) guidelines state that ipratropium bromide provides additive benefit to short-acting beta-agonists in moderate or severe exacerbations in the emergency care setting 3
- Adding ipratropium to albuterol produces modest but clinically meaningful improvement in severe bronchospasm, according to the American Heart Association 2
Dosage and Administration
- The recommended dosing for moderate to severe acute bronchospasm is 0.5 mg ipratropium mixed with 2.5-5 mg albuterol every 20 minutes for 3 doses, then as needed 2
- Albuterol 2-4 puffs (MDI) or 2.5 mg (nebulizer) every 20 minutes for up to 3 doses can be used for mild acute bronchospasm 1, 2
Monitoring and Adjustment
- If using albuterol more than twice weekly for symptom relief, this indicates inadequate control and requires adjustment of controller medications, as recommended by the American College of Chest Physicians 1, 4
- Peak flow monitoring helps assess response to therapy and detect early signs of worsening 1
- Consider hospital admission if patients have temperature >37.8°C, heart rate >100/min, respiratory rate >24/min, or oxygen saturation <90% 2
