Resuming Anticoagulants After Hematuria in Patients with Impaired Renal Function
Immediate Management and Anticoagulation Resumption
- The American College of Cardiology recommends that anticoagulation can often be continued without interruption in patients with minor, self-limiting hematuria, though even minor bleeding predicts subsequent major bleeding risk 1
- The British Society of Gastroenterology and European Society of Gastrointestinal Endoscopy recommend resuming DOACs 24 hours after hemostasis for low bleeding risk scenarios, and 48-72 hours after hemostasis for high bleeding risk scenarios 2, 3
Specific Recommendations by Anticoagulant Type and Renal Function
- The American College of Cardiology recommends resuming warfarin at usual maintenance dose once hemostasis is established, and considering bridging with LMWH if high-dose vitamin K was used for reversal 1, 5
- For dabigatran, the recommended dose is contraindicated if CrCl <30 mL/min, and a reduced dose of 110-150 mg once daily is recommended for the first 2-3 days, then 150 mg twice daily for CrCl 30-50 mL/min 2, 3, 6
- For rivaroxaban, a reduced dose of 10 mg once daily is recommended for the first 2-3 days, then 20 mg once daily 3
- Apixaban is the preferred DOAC in moderate renal impairment due to lower renal clearance, and a reduced dose of 2.5 mg twice daily is recommended for the first 2-3 days, and permanently if the patient has ≥2 of: age ≥80 years, weight ≤60 kg, serum creatinine ≥1.5 mg/dL 3, 7
Monitoring and Prevention of Recurrent Hematuria
- The Journal of the American College of Cardiology recommends monitoring hemoglobin every 4-6 hours initially until stable, and reassessing renal function before resuming DOACs 4
- The British Society of Gastroenterology and European Society of Gastrointestinal Endoscopy recommend preferring LMWH or DOACs over unfractionated heparin or warfarin to minimize recurrence risk 2
- The Gut journal recommends balancing thrombotic risk against bleeding risk when determining timing of resumption, and considering patient's CHADS₂ score and indication for anticoagulation 2, 3, 4
