Effectiveness of Progesterone-Only Contraceptives for Abnormal Uterine Bleeding

Types of Progesterone-Only Contraceptives and Their Effectiveness

  • The American College of Cardiology recommends levonorgestrel-releasing intrauterine devices as a highly effective option for managing abnormal uterine bleeding, reducing menstrual blood loss by 71-95% in cases of abnormal uterine bleeding 1, 2
  • Levonorgestrel intrauterine devices (20 μg/day) are the most effective progesterone-only option for abnormal uterine bleeding, with efficacy comparable to endometrial ablation 1, 2
  • The Centers for Disease Control and Prevention recommends that subdermal implants can help manage abnormal uterine bleeding but may themselves cause irregular bleeding patterns, especially during the first year of use 3

Management Algorithm for Abnormal Uterine Bleeding with POCs

  • The American Academy of Family Physicians recommends ruling out pregnancy, structural causes, and malignancy before attributing bleeding to hormonal imbalance, and considering the PALM-COEIN classification system to identify the cause of abnormal uterine bleeding 4, 5
  • For women with ovulatory dysfunction bleeding, the American College of Cardiology recommends LNG-IUD (20 μg/day) as the most effective option 1, 2

Special Populations

  • The American College of Cardiology recommends that for women on antiplatelet therapy who develop abnormal uterine bleeding, LNG-IUDs are particularly effective and can reduce menstrual blood loss by 71-95% 1, 2
  • The Centers for Disease Control and Prevention recommends that for women with persistent bleeding despite POC use who find it unacceptable, alternative contraceptive methods should be offered 3

Managing Prolonged Menstrual Bleeding with Progesterone

Treatment Options

  • NSAIDs are effective for unscheduled spotting or light bleeding, as well as heavy or prolonged bleeding, according to the MMWR Recommendations and Reports 6
  • For women who experience cyclic heavy bleeding related to the menstrual cycle, progesterone therapy can be particularly effective, as noted by Hepatology 7
  • Oral progesterone for 21 days per month is effective for reducing menstrual blood loss, as recommended by the MMWR Recommendations and Reports 8
  • For women with heavy bleeding due to fibroids, the LNG-IUD has shown superior results to progesterone therapy, according to the Journal of the American College of Radiology 9
  • If bleeding persists or is unacceptable despite treatment, alternative methods should be considered, as suggested by the MMWR Recommendations and Reports 6

Management of Prolonged Menstrual Bleeding

Treatment Options

  • For women with prolonged bleeding who don't respond to initial treatment, a second course of progesterone may be considered after appropriate evaluation for underlying causes, as recommended by the Centers for Disease Control and Prevention (CDC) 10
  • NSAIDs can reduce bleeding volume in women with heavy menstrual bleeding, with a treatment duration of 5-7 days, according to the CDC 10, 11

Contraindications and Precautions

  • Progesterone should not be used in women with known or suspected pregnancy, current or history of thromboembolism, or undiagnosed vaginal bleeding, as stated by the Infectious Diseases Society of America 12 and the CDC 11
  • Women with liver disease should also avoid progesterone, as advised by the American College of Radiology 13

Evaluation and Follow-up

  • If bleeding persists after treatment or recurs frequently, further evaluation for underlying conditions such as fibroids, polyps, or endometrial hyperplasia is warranted, as recommended by the CDC 10 and the American College of Radiology 13

Progesterone Therapy for Heavy Menstrual Bleeding

Effective Treatment Options

  • The American College of Cardiology recommends the levonorgestrel IUD (LNG-IUS) as the most effective medical treatment for heavy menstrual bleeding, reducing menstrual blood loss by 71-95% 14
  • The American College of Cardiology suggests that tranexamic acid during menstruation (5-7 days) is highly effective for reducing menstrual blood loss 15
  • The American Heart Association recommends avoiding NSAIDs and tranexamic acid in women with cardiovascular disease or thrombotic risk due to MI and thrombosis associations 14

Critical Evaluation and Special Populations

  • The American College of Cardiology advises urgent evaluation if bleeding saturates a large pad/tampon hourly for ≥4 hours, and recommends reassessing indication for ongoing antiplatelet therapy in women on antiplatelet therapy with heavy bleeding 14
  • The American College of Cardiology recommends the LNG-IUS as a particularly effective treatment option for women on anticoagulation, reducing bleeding by 71-95% 14
