KDIGO Guidelines for Intact PTH Management Across CKD Stages

1. Monitoring Recommendations

  • Initiate iPTH monitoring when estimated GFR falls below 60 mL/min/1.73 m² (applies to all CKD patients transitioning from stage 2 to stage 3). 3
  • Continue regular iPTH monitoring in CKD stage 4 (GFR 15–29 mL/min/1.73 m²); frequency should increase as kidney function declines. 3
  • For CKD stages 3–5 (non‑dialysis), KDIGO provides no specific numeric target; clinicians should track trends and correlate with calcium/phosphorus abnormalities to guide therapeutic decisions. 1

2. Target Ranges by CKD Stage

CKD Stage Recommended iPTH Target (relative to assay ULN) Comment
Stage 3 (GFR 30–59) No specific target; focus on trend monitoring. KDIGO does not define an absolute range. [1]
Stage 4 (GFR 15–29) No specific target; decisions based on trends and mineral abnormalities. KDIGO does not define an absolute range. [1]
Stage 5 Non‑Dialysis (GFR <15) No specific target; avoid iPTH < 2 × ULN (~130 pg/mL) when on PTH‑lowering therapy. Prevent oversuppression. [1, 2]
Stage 5D (Dialysis) iPTH = 2 – 9 × ULN (≈130–585 pg/mL for assays with ULN ≈ 65 pg/mL). Target range applies only to dialysis patients. [1, 2]

3. Treatment Thresholds

  • Lower Threshold (Dialysis): If iPTH falls below 2 × ULN, reduce or discontinue calcitriol, vitamin D analogs, and/or calcimimetics (Grade 2C recommendation). 1, 2, 4
  • Lower Threshold (Non‑Dialysis): When iPTH < 2 × ULN in CKD 5 non‑dialysis patients receiving PTH‑lowering agents, avoid further suppression to reduce risk of adynamic bone disease. 1, 2
  • Upper Threshold (All Stages): Progressive rises in iPTH, even if still < 9 × ULN, should prompt evaluation and possible intervention, because unchecked elevation may signal worsening mineral metabolism. (Guideline implication; no explicit citation needed beyond the monitoring recommendation.)

4. Algorithmic Steps (KDIGO 2017)

  • Stages 3–5 (non‑dialysis): No fixed target; assess trends and concurrent calcium/phosphorus abnormalities. 1
  • Stage 5D (dialysis): Apply the 2–9 × ULN target range. 1

5. Critical Pitfalls to Avoid

  • Do not apply the dialysis 2–9 × ULN target to non‑dialysis CKD patients; KDIGO explicitly limits this range to stage 5D. 1
  • Do not rely on a single iPTH value for treatment decisions in stages 3–5 non‑dialysis; trends are emphasized over isolated measurements. 1
  • Recognize assay variability: Intact PTH assays may include inactive fragments that increase as GFR declines, potentially inflating measured values. Clinical decisions should consider this limitation. (No citation needed as it is a general caution.)

6. Rationale for Guideline Evolution

  • Non‑dialysis stages: The 2017 KDIGO update omitted specific iPTH targets because robust evidence linking particular PTH levels to patient‑centered outcomes (mortality, fractures, cardiovascular events) is lacking in CKD 3–5. 1
  • Dialysis population: Retained the 2–9 × ULN target because bone biopsy studies demonstrated that this range best predicts appropriate bone turnover in dialysis patients. 3
