Severe Pelvic Inflammatory Disease Management

Diagnosis and Initial Management

  • The American College of Obstetricians and Gynecologists recommends initiating empiric broad-spectrum antibiotics immediately in patients with severe pelvic inflammatory disease (PID), without waiting for culture results, to prevent irreversible tubal damage and infertility 1, 2, 5
  • The Centers for Disease Control and Prevention (CDC) suggests that patients with severe PID, such as those with bilateral pyosalpinx, should be hospitalized for intravenous antibiotic therapy and close monitoring 3, 4, 5
  • The CDC recommends obtaining cervical cultures for N. gonorrhoeae and NAAT for C. trachomatis in all patients with PID, as well as a pregnancy test to exclude ectopic pregnancy 4, 5

Antibiotic Regimens

  • The American College of Obstetricians and Gynecologists recommends a parenteral regimen of ceftriaxone 1-2g IV daily, plus doxycycline 100mg IV/PO twice daily, plus metronidazole 500mg IV every 8 hours for enhanced anaerobic coverage in patients with severe PID 5
  • The CDC suggests transitioning to oral doxycycline 100mg twice daily to complete 14 days total after 24 hours of clinical improvement 5

Follow-Up and Prevention

  • The American College of Obstetricians and Gynecologists recommends daily clinical assessment until sustained improvement, as well as repeat imaging in 48-72 hours if no improvement to assess for abscess requiring drainage 5
  • The CDC suggests repeat screening for STIs at 4-6 weeks, as well as counseling regarding reproductive sequelae including increased risk of infertility, ectopic pregnancy, and chronic pelvic pain 5
