Iron Deficiency Anemia Treatment Guidelines
Introduction to Iron Dosage
- The recommended dosage of elemental iron for patients with iron deficiency anemia is 50-100 mg of elemental iron once daily, as suggested by the Gut guideline 1
- Different iron salt preparations contain varying amounts of elemental iron, with ferrous sulfate containing 65 mg, ferrous fumarate containing 106 mg, and ferrous gluconate containing 38 mg of elemental iron per 325 mg tablet 1
Patient-Specific Dosage Recommendations
- For initial therapy, 50-100 mg of elemental iron once daily is recommended, which typically corresponds to one 325 mg ferrous sulfate tablet daily 1
- For patients with Inflammatory Bowel Disease, no more than 100 mg elemental iron per day is recommended 4
- For patients with Chronic Kidney Disease, 200 mg elemental iron daily, divided into 2-3 doses, is recommended 3
Iron Absorption Optimization
- Iron is best absorbed when taken without food or other medications, and on an empty stomach 3
- Avoiding calcium-containing foods/supplements can help reduce interference with iron absorption 3
Treatment Monitoring and Adjustment
- Assess adequacy of response after 1 month, looking for a hemoglobin rise of ≥1.0 g/dL, normalization of ferritin and transferrin saturation 5
- If response is inadequate, consider increasing the daily dose or switching to twice-daily dosing, trying an alternate oral iron preparation, or considering intravenous iron 1
- Continue iron replacement for approximately 3 months after normalization of hemoglobin levels to ensure adequate repletion of iron stores 1
