Treatment of MRSA Infections in Children

Introduction to MRSA Treatment

  • The American Academy of Pediatrics and the Infectious Diseases Society of America recommend IV vancomycin as the first-line treatment for serious MRSA infections in children, while clindamycin or trimethoprim-sulfamethoxazole can be used for stable patients without bacteremia 1, 2, 3

Treatment by Infection Type and Severity

  • For minor, localized superficial MRSA infections without deeper tissue involvement, purulent drainage, or systemic illness, the Infectious Diseases Society of America recommends topical mupirocin 2% ointment applied three times daily for 5-10 days 4
  • For uncomplicated skin and soft tissue MRSA infections, incision and drainage is the mainstay of therapy, with antibiotics being adjunctive, and the Infectious Diseases Society of America recommends adding antibiotics if any of the following are present: severe or extensive disease, rapid progression, signs of systemic illness, immunosuppression, or extremes of age 5
  • The Infectious Diseases Society of America recommends oral antibiotic options for outpatient treatment of MRSA infections, including clindamycin, trimethoprim-sulfamethoxazole, and linezolid, with specific dosing recommendations for each 1, 3, 4, 5

Serious/Invasive Infections

  • The Infectious Diseases Society of America recommends IV vancomycin as the first-line treatment for serious MRSA infections, including bacteremia, pneumonia, and osteomyelitis, with a minimum treatment duration of 2 weeks for uncomplicated bacteremia and 4-6 weeks for complicated bacteremia 1, 2, 3
  • For osteomyelitis, the Infectious Diseases Society of America recommends surgical debridement and drainage, along with a minimum 8-week course of antibiotics, including IV vancomycin or daptomycin 1, 2, 3

Specific Infection Types

  • For septic arthritis, the Infectious Diseases Society of America recommends drainage or debridement of the joint space, along with a minimum 3-4 weeks of therapy 2
  • For pneumonia with empyema, the Infectious Diseases Society of America recommends IV vancomycin with drainage procedures 1, 3

Critical Management Principles

  • The Infectious Diseases Society of America emphasizes the importance of source control, including drainage of abscesses, removal of central venous catheters, and debridement of osteomyelitis or necrotic tissue 6, 1, 3, 7
  • The Infectious Diseases Society of America recommends monitoring clinical response within 48-72 hours of initiating therapy and adjusting antibiotics based on culture and susceptibility results 4, 7

Common Pitfalls and Caveats

  • The Infectious Diseases Society of America warns against using clindamycin when local resistance rates are ≥10% or unknown, and recommends choosing alternative antibiotics such as trimethoprim-sulfamethoxazole or doxycycline/minocycline instead 1, 2, 3, 5
