Levetiracetam Use in Status Epilepticus, Special Populations, and Safety Profile

Status Epilepticus Dosing

  • Adult and adolescent benzodiazepine‑refractory status epilepticus: a loading dose of 60 mg/kg IV (maximum 4.5 g) is recommended; this dose terminates seizures in roughly 47 % of cases and shows efficacy comparable to fosphenytoin and valproate. 1
  • Pediatric status epilepticus (all ages): a loading dose of 40 mg/kg IV bolus (maximum 2.5 g) is advised. [2][3]1
  • Maintenance after non‑convulsive status epilepticus in children: 15 mg/kg IV every 12 h (maximum 1.5 g per dose). [2][3]1
  • Maintenance after convulsive status epilepticus in children: either 30 mg/kg IV every 12 h or an incremental prophylactic regimen of an additional 10 mg/kg (total 20 mg/kg) every 12 h, each with a ceiling of 1.5 g per dose. [2][3]1
  • Clarification on loading versus maintenance dosing: maintenance doses should never be used as the initial loading dose for status epilepticus; the appropriate loading range is 40–60 mg/kg, with the strongest current evidence supporting 60 mg/kg in adults. 1

Prophylaxis in Special Populations

  • CAR‑T cell therapy recipients with central‑nervous‑system disease or prior seizures: administer 10 mg/kg IV every 12 h (maximum 500 mg per dose) for a total duration of 30 days. 1
  • Patients with newly diagnosed brain tumors who have never experienced a seizure: prophylactic levetiracetam is not recommended because it does not improve seizure‑free survival or reduce the incidence of a first seizure at six months. 4
  • If antiseizure medication becomes necessary in brain‑tumor patients (e.g., after a breakthrough seizure): levetiracetam may be preferred over older agents owing to its superior tolerability profile. 4

Common Adverse Effects

  • The most frequently reported adverse events with levetiracetam are somnolence, dizziness, asthenia, and headache. 1

Monitoring Recommendations

  • Behavioral monitoring: patients with a known psychiatric history should be observed closely for emerging mood or behavioral disturbances while on levetiracetam. 2

Guidance on Dose Selection

  • Loading dose selection for status epilepticus: avoid using standard maintenance doses (e.g., 10 mg/kg BID) as the initial loading dose; instead, employ the higher loading regimen of 40–60 mg/kg as outlined above. 1
