Management of Tacrolimus Neurotoxicity
Clinical Presentation and Diagnosis
- Neurotoxicity can manifest with a wide spectrum of symptoms, ranging from mild to severe, including tremors, paresthesias, insomnia, headache, anxiety, agitation, language disturbances, and motor dysfunction, according to the American Academy of Dermatology 1, 2, 3, 4
- The National Comprehensive Cancer Network recommends neurology consultation at the first sign of neurotoxicity 4
Management Algorithm
- Obtain tacrolimus trough levels immediately and perform neurological assessment to grade severity of symptoms, as recommended by the American College of Chest Physicians 5, 4, 6
- For moderate to severe symptoms, obtain MRI of brain with and without contrast and consider EEG for seizure activity assessment, according to the National Comprehensive Cancer Network 4
- Reduce tacrolimus dose to achieve lower therapeutic levels and monitor tacrolimus levels more frequently until stabilized, as recommended by the American College of Chest Physicians 5
Special Considerations
- The National Comprehensive Cancer Network recommends avoiding medications that can cause CNS depression unless needed for seizure management and considering antifungal prophylaxis in patients receiving steroids for treatment of neurotoxicity 4
- Use a fast taper when improvement occurs if steroids are given for management of neurotoxicity, as recommended by the National Comprehensive Cancer Network 4
Prevention Strategies
- Regular monitoring of tacrolimus blood levels is essential, with recommended trough levels between 5-15 ng/mL for most transplant recipients, according to the American College of Chest Physicians 5
- Monitor serum potassium, glucose, renal function, and hepatic function to detect tacrolimus-induced abnormalities, as recommended by the American College of Chest Physicians 5, 6
Key Pitfalls to Avoid
- Prompt management is critical for reducing long-term morbidity when neurotoxicity is suspected, according to the National Comprehensive Cancer Network 7
Management of Tacrolimus Neurotoxicity
Management Algorithm
- For moderate to severe neurotoxicity, the American Society of Clinical Oncology recommends consideration of temporary discontinuation of tacrolimus and initiation of corticosteroids (methylprednisolone 1-2 mg/kg) for significant neurological symptoms, with a strength of evidence based on clinical guidelines 8, 9
- Alternative immunosuppression strategies, as suggested by the European Society for Medical Oncology, include considering switching to cyclosporine in cases of severe neurotoxicity, with mycophenolate mofetil as a calcineurin inhibitor-sparing agent due to its lack of neurotoxic effects 9
