Adjuvant Therapy for Gastric Leiomyosarcoma

Introduction to Adjuvant Therapy

  • The American College of Oncology recommends doxorubicin-based chemotherapy as adjuvant therapy for high-risk gastric leiomyosarcoma patients, which may be considered following complete surgical resection, with high-risk defined as high-grade, deep, or >5 cm tumors 1

Adjuvant Chemotherapy Options

  • The National Comprehensive Cancer Network recommends doxorubicin-based regimens as first-line treatment for high-risk patients, with single-agent doxorubicin considered standard first-line treatment 1
  • Doxorubicin plus ifosfamide may be considered when a higher response rate is desired in patients with good performance status, although ifosfamide has shown less convincing activity in leiomyosarcoma 1, 3
  • Doxorubicin plus dacarbazine is preferred over regimens containing ifosfamide for leiomyosarcoma, due to ifosfamide's less convincing activity in this tumor type 1, 4

Decision-Making for Adjuvant Therapy

  • Adjuvant chemotherapy is not standard treatment for all soft tissue sarcomas but can be proposed for high-risk patients, with the decision made after multidisciplinary evaluation considering tumor grade, size, depth of invasion, and margin status 1, 2
  • The European Society for Medical Oncology suggests that tumor grade, with high-grade tumors benefiting most from adjuvant chemotherapy, is a key factor in decision-making 2

Second-line and Subsequent Treatment Options

  • Trabectedin has proven effective in leiomyosarcoma after failure of anthracycline-based therapy, and gemcitabine with or without docetaxel has shown activity in leiomyosarcoma 1, 4
  • Dacarbazine has demonstrated activity as second-line therapy in leiomyosarcoma, and gemcitabine plus dacarbazine has shown improved overall survival and progression-free survival over dacarbazine alone in a randomized trial 1, 3, 4

Special Considerations

  • Approximately 50% of uterine leiomyosarcomas express estrogen and/or progesterone receptors, suggesting potential hormone sensitivity that may extend to gastric leiomyosarcomas, although this is based on a guideline from Praxis Medical Insights and should be considered with caution 5

Treatment Algorithm

  • Risk assessment based on tumor grade, size, depth, and margin status is crucial for determining the need for adjuvant chemotherapy, with high-risk patients considered for doxorubicin-based chemotherapy 1, 2, 4, 6

Follow-up Recommendations

  • High-grade patients should be followed every 3-4 months in the first 2-3 years, then twice a year up to the fifth year, and once a year thereafter, with follow-up including appropriate imaging to detect recurrence 6

Pitfalls and Caveats

  • The value of adjuvant chemotherapy remains controversial with conflicting study results, though meta-analyses suggest benefit in high-risk patients, and treatment decisions should be made in centers with expertise in sarcoma management 1, 2, 4
