Contraindications and Cautions for Cabotegravir PrEP

Critical Pre-Initiation Requirements

  • Comprehensive HIV testing, including a fourth- or fifth-generation antigen/antibody test and an HIV RNA test with a lower limit of quantification ≤50 copies/mL, is mandatory before the first injection, as recommended by the American Medical Association, to prevent integrase inhibitor resistance and ensure accurate diagnosis 1
  • If clinical suspicion exists for acute HIV infection, cabotegravir should not be initiated until HIV RNA results confirm negative status, according to the Centers for Disease Control and Prevention guidelines 2, 3

Important Cautions and Special Populations

Drug Interactions Requiring Dose Adjustment

  • Rifabutin coadministration requires dose adjustment when used with cabotegravir, as per the World Health Organization guidelines 1
  • All potent UGT1A1 inducers should be avoided, as they reduce cabotegravir concentrations, according to the European Medicines Agency recommendations 1

Anatomical Considerations

  • Cabotegravir should be used with caution in individuals with gluteal implants or fillers, as the intramuscular injection site may be compromised, as noted by the American College of Obstetricians and Gynecologists 1

Renal Considerations

  • Baseline serum creatinine and estimated creatinine clearance are required before initiation, as recommended by the National Kidney Foundation 2, 3

Mandatory Baseline Testing Before First Injection

  • Hepatitis B surface antigen, hepatitis C antibody, serum creatinine, estimated creatinine clearance, pregnancy test, and STI screening are required before the first injection, according to the Centers for Disease Control and Prevention guidelines 2, 3

Ongoing Monitoring Requirements

  • HIV testing must be repeated before every subsequent injection using a point-of-care rapid antibody test plus laboratory antigen/antibody test, as recommended by the World Health Organization 1

Medical Necessity of Cabotegravir Injection for HIV Pre-Exposure Prophylaxis

Indication and Approval Status

  • Long-acting cabotegravir received regulatory approval for prevention of sexual acquisition of HIV across populations based on large randomized clinical trials (evidence rating: AIa) 4

Dosing Schedule and Medical Necessity

  • Injections by gluteal administration at 600 mg (3 mL) are recommended with the first 2 injections separated by 4 weeks and subsequent injections by 8 weeks (evidence rating: AIa) 5, 6
  • The patient's planned schedule (initial injection followed by 1-month and then every-other-month injections) matches guideline recommendations 5

Safety Considerations

  • Common side effects include injection site reactions, which occurred in up to 81% of trial participants but typically diminish in severity over time 7

Implementation Protocol

  • The patient should have HIV testing (ideally antigen/antibody combination test AND HIV RNA test) before initiating cabotegravir and before each subsequent injection 7
  • The patient should have a 1-month supply of an appropriate tenofovir-based oral PrEP agent available for oral bridging in case of injection delays of 7 days or more 6
  • If injections are delayed by 8 or more weeks, "reloading" with two injections 4 weeks apart is recommended before returning to every-8-week dosing 7

Common Pitfalls to Avoid

  • Inadequate HIV testing before initiation could miss early HIV infection 8

Laboratory Testing at 1 Month After First Cabotegravir Injection

Mandatory HIV Testing Protocol

  • The American Medical Association recommends performing HIV testing using both a point-of-care rapid HIV antibody test and a laboratory-based fourth- or fifth-generation antigen/antibody test at the 1-month follow-up visit, as cabotegravir can delay HIV seroconversion and mask early infection 9
  • A point-of-care rapid HIV antibody test should be performed at the visit, in addition to a laboratory-based fourth- or fifth-generation antigen/antibody combination test sent for processing, to ensure accurate detection of HIV infection 9
  • The Centers for Disease Control and Prevention advises against routinely ordering HIV RNA testing at follow-up visits for cabotegravir PrEP, as it has a low positive predictive value and false-positive results can have significant negative consequences 9
  • HIV RNA testing should be reserved for initial PrEP initiation/resumption after hiatus, or when there is clinical suspicion of acute HIV infection, rather than routine monitoring 9
  • The World Health Organization recommends assessing STI screening at all exposed mucosal sites based on sexual history and exposures, in addition to HIV testing, at the 1-month follow-up visit 10
  • Adherence assessment to the injection schedule should be performed at the 1-month follow-up visit to ensure patients are following the recommended treatment regimen 9

Cabotegravir Indication and Management

Patient Selection and Testing

  • The patient's history of unprotected sexual encounters establishes high-risk status, meeting the core indication for PrEP, as recommended by the American Medical Association, with a high-risk status defined as having unprotected sexual encounters 11, 12
  • Combined HIV antibody and antigen testing, including fourth-generation antigen/antibody testing before initiation and before each injection, is mandatory to prevent integrase inhibitor resistance if undetected HIV infection exists, as suggested by the Centers for Disease Control and Prevention 11, 12

Baseline Laboratory Work and Safety Considerations

  • Baseline laboratory work, including serum creatinine, estimated creatinine clearance, hepatitis B surface antigen, hepatitis C antibody, and STI screening, is required before initiating cabotegravir, as recommended by the World Health Organization 11
  • Hepatitis B surface antigen and hepatitis C antibody testing are necessary to assess the patient's liver health before starting cabotegravir, as suggested by the American Association for the Study of Liver Diseases 11
