Sedation for Cardioversion in Conscious SVT Patients

Hemodynamic Status Determines Sedation Timing

  • Sedation is required for conscious patients undergoing cardioversion for SVT, with the only exception being extremely unstable patients where cardioversion should not be delayed, as recommended by the American Heart Association 1, 2
  • Adequate sedation or anesthesia must be administered before cardioversion in all stable conscious patients, with a Class I recommendation from the American College of Cardiology/American Heart Association/Heart Rhythm Society (ACC/AHA/HRS) guidelines 3, 4
  • Establish IV access before cardioversion to facilitate sedation administration, as suggested by the American Heart Association 1, 2

Sedation in Hemodynamically Unstable Patients

  • Immediate sedation should still be given to hypotensive but conscious patients before cardioversion, according to the European Society of Cardiology (ESC) guidelines 5
  • Do not delay cardioversion if the patient is extremely unstable, as stated by the American Heart Association (AHA) ACLS guidelines 1, 2

Energy Dosing for SVT

  • Initial energy of 50-100 J is typically sufficient for SVT cardioversion, as recommended by the American Heart Association 1, 2
  • Increase dose in stepwise fashion if initial shock fails, as suggested by the American Heart Association 1, 2
