Exercise Guidelines for NYHA Class 2-3 Heart Failure Patients

Primary Exercise Modality: Cycle Ergometry

  • The European Society of Cardiology recommends cycle ergometer training as the foundation of exercise prescription for NYHA class 2-3 heart failure patients, allowing for precise control of workloads, exact reproducibility of prescribed intensity, and continuous monitoring of heart rate, rhythm, and blood pressure during exercise 1, 2
  • Cycle ergometer training should be performed at 60-80% of peak heart rate for 20-40 minutes, 3-5 times per week, combined with low-intensity resistance training 1, 2

Exercise Prescription Parameters

  • The American Heart Association recommends aerobic training intensity of 60-80% of peak heart rate or 60-70% of peak VO2, with a duration of 20-40 minutes per session, and a frequency of 3-5 times per week 1, 3
  • The American College of Cardiology recommends a program length of at least 8 weeks, with optimal benefits continuing up to 6 months 3

Resistance Training Component

  • The American Heart Association recommends adding low-intensity, high-repetition resistance training to the aerobic program, with an intensity of 50-70% of 1-repetition maximum, 10-15 repetitions per set, and 1-2 sets per exercise, targeting major muscle groups of upper and lower extremities 6
  • Resistance training produces significant strength increases, allowing some patients to reduce dependence on walking aids, and addresses skeletal muscle weakness that compounds functional decline in heart failure 6

Critical Safety Requirements

  • The European Society of Cardiology recommends that patients meet strict stability criteria before starting exercise, including stable symptoms for at least 2 weeks, no resting symptoms or postural hypotension, stable fluid balance, and no evidence of congestion 7
  • Exercise is contraindicated when systolic blood pressure is <80 mmHg, resting heart rate is <50 or >100 bpm, dressing and body care cause symptoms, or NYHA class IV with dyspnea at rest 7, 8

Expected Outcomes

  • Patients completing this exercise prescription typically achieve peak VO2 improvements of 12-31%, and significant improvements in quality of life, particularly exercise tolerance and general health domains 3
