Escitalopram Metabolism and Interactions

Pharmacokinetics and Metabolism

  • The American Academy of Child and Adolescent Psychiatry notes that escitalopram has a minimal effect on CYP3A4, with a minimal inhibitory effect on this enzyme 1
  • The escitalopram has the lowest effect on CYP450 isoenzymes compared to other SSRIs, including a minimal inhibitory effect on CYP3A4, as reported by the Journal of the American Academy of Child and Adolescent Psychiatry 1

Clinical Implications

  • The coadministration of ritonavir, a potent CYP3A4 inhibitor, with escitalopram does not significantly affect the pharmacokinetics of escitalopram, demonstrating that CYP3A4 inhibition does not alter the elimination of the drug 1
  • No dose adjustment is required when escitalopram is coadministered with CYP3A4 inhibitors, unlike inhibitors of CYP2C19 such as omeprazole or cimetidine 1
