Calcium Channel Blocker Combination Therapy

Rationale Against Combination

  • Major hypertension guidelines, including those from the American College of Cardiology and the European Society of Cardiology, do not recommend combining two different calcium channel blockers for blood pressure management 1, 2, 3.
  • The 2013 ESH/ESC Guidelines explicitly outline preferred, useful, and not-recommended drug combinations, and combining different calcium channel blocker subclasses is notably absent from recommended strategies 3, 4.

Pharmacological Concerns

  • Diltiazem has prominent negative inotropic and chronotropic effects, affecting both AV conduction and sinus node function, which can be problematic when combined with another calcium channel blocker 5.

Preferred Alternative Strategies

  • The American College of Cardiology and the European Society of Cardiology recommend using a dihydropyridine calcium channel blocker, such as cilnidipine, in combination with an ACE inhibitor or ARB, as a preferred combination with proven cardiovascular benefit 1, 2, 3, 4.
  • Another well-established effective combination is a dihydropyridine calcium channel blocker plus a thiazide diuretic 1, 2, 3.

Critical Clinical Pitfalls to Avoid

  • The American Heart Association advises against using diltiazem in patients with heart failure with reduced ejection fraction under any circumstances 7, 1, 2, 6.
