Immediate Treatment for Epiglottitis

Initial Assessment and Airway Management

  • Epiglottitis presents with sudden onset of severe sore throat, odynophagia, and potential rapid progression to airway obstruction, requiring immediate discussion with specialists for emergency airway intervention 1
  • Respiratory symptoms and hypoxemia should prompt immediate discussion with an intensivist and rapid transfer to an ICU or burn center 2
  • Blood cultures should be obtained to identify the causative organism, often Haemophilus influenzae, in patients with epiglottitis 1

Immediate Airway Intervention

  • Prepare for difficult airway management with appropriate equipment and personnel, including supraglottic airway devices, videolaryngoscope, and surgical airway equipment for emergency cricothyroidotomy 3

Approach to Airway Management

  • Maintain oxygenation as the primary goal, and position the patient appropriately, typically upright position if conscious, to secure the airway 3
  • Have difficult airway equipment immediately available, including videolaryngoscope if available, to facilitate airway management 4
  • Follow a structured approach similar to difficult airway algorithms with plans for failed intubation, to ensure prompt and effective airway intervention 3

Common Pitfalls to Avoid

  • Attempting throat examination with tongue depressors may precipitate complete airway obstruction, and should be avoided in patients with epiglottitis 1
  • Failing to prepare for difficult airway management before attempting intubation can lead to significant complications, and should be avoided 3
