Treatment of IgM Deficiency
Diagnosis and Clinical Presentation
- Laboratory evaluation should include measurement of immunoglobulin levels, specific antibody production, and evaluation of lymphocyte subpopulations, as recommended by the American Academy of Allergy, Asthma, and Immunology 1
Treatment Approach
Immunoglobulin Replacement Therapy
- IVIG therapy should be initiated for patients with recurrent bacterial infections, as recommended by the American Academy of Allergy, Asthma, and Immunology 1, 2
- IVIG therapy should also be considered for patients with life-threatening infections, documented bacterial infection with insufficient response to antibiotic therapy, as recommended by the European Society for Immunodeficiencies 2
- Monthly IVIG treatment should be continued until immunoglobulin levels reach adequate levels (≥400 mg/dl for IgG), with IgG trough levels monitored monthly during treatment, as recommended by the International Patient Organization for Primary Immunodeficiencies 2
Special Considerations
- Monitor frequency of infections rather than relying solely on serum immunoglobulin levels to assess treatment efficacy, as recommended by the American College of Allergy, Asthma, and Immunology 2
- For patients with milder presentations, prophylactic antibiotics can be considered as an alternative first-line treatment option, as recommended by the Infectious Diseases Society of America 2
