Treatment of Hand Burns
Initial Assessment and Management
- Hand burns require immediate cooling with clean running water to limit tissue damage and reduce pain, with cooling times of 5-20 minutes, which has been shown to decrease the need for subsequent care, as recommended by the American Heart Association 1, 2, 3, 4
- Remove all jewelry before swelling occurs to prevent vascular compromise, and administer over-the-counter pain medications such as acetaminophen or NSAIDs, according to the American College of Cardiology 2, 5, 6
Burn Classification and Referral Criteria
- Hand burns are classified as superficial (first-degree), partial-thickness (second-degree), or full-thickness (third-degree), with partial or full-thickness burns requiring evaluation at a specialized burn center due to high risk of functional disability, as stated by the American Burn Association 1, 2, 7, 8
Treatment Based on Burn Depth
- For superficial burns, continue cooling with clean running water, apply petrolatum or antibiotic ointment, and cover with a clean, non-adherent dressing, as recommended by the American College of Surgeons 1, 3, 5, 6
- For partial-thickness burns, loosely cover with a clean, non-adherent dressing, refer to a burn specialist or burn center immediately, and clean the wound with tap water or isotonic saline if transfer is delayed, according to the American Burn Association 1, 5, 6, 7, 9
- For full-thickness burns, cover with a clean, dry, non-adherent dressing, and immediate referral to a burn center is mandatory, with monitoring for signs of compartment syndrome requiring escharotomy, as stated by the American College of Surgeons 1, 5, 6, 7, 8
Common Pitfalls to Avoid
- Do not use ice directly on burns, apply butter or oil, or delay referral for partial or full-thickness hand burns, as this can lead to permanent functional impairment, according to the American Burn Association 1, 2, 3, 4, 7, 8, 9
- Monitor children closely for hypothermia during cooling of burns, as recommended by the American Academy of Pediatrics 1, 2
