Erectile Dysfunction Treatment

Diagnosis and Treatment Recommendations

  • The American Urological Association recommends a PDE5 inhibitor, such as tadalafil, as first-line treatment for erectile dysfunction, with careful consideration of nitrate therapy, in patients with cardiovascular comorbidities 1, 2, 3
  • Serum total testosterone measurement is indicated to rule out testosterone deficiency, as recommended by the Endocrine Society and the American Urological Association 1, 4
  • The absence of morning erections suggests an organic rather than purely psychogenic cause of erectile dysfunction, according to the European Association of Urology 1
  • Optimal doses for PDE5 inhibitors are:
Medication Optimal Dose
Sildenafil 25mg to 100mg as needed [5]
Tadalafil 5mg to 20mg as needed, or 2.5-5mg daily [5]
Vardenafil 5-20mg as needed [1]
Avanafil 50-200mg as needed [1, 5]
  • Ensure patient has tried sildenafil at maximum dose (100mg) on at least 5 separate occasions, as recommended by the National Comprehensive Cancer Network 6

Alternative Treatment Options

  • The European Association of Urology considers vacuum erection devices (VEDs) as an alternative non-pharmacological option for patients who cannot discontinue nitrates, with 90% initial efficacy 2, 4
  • Intracavernosal injection therapy (e.g., alprostadil) is another option if PDE5 inhibitors are contraindicated, as suggested by the European Association of Urology and the American Urological Association 2, 3
  • Penile prostheses are reserved for patients who have failed or cannot use less invasive treatments and have high satisfaction rates but are irreversible, as recommended by The American Urological Association 3

Comorbid Condition Management

  • The American Diabetes Association recommends optimizing glycemic control, blood pressure, and lipid management to improve erectile function 4
  • The European Society of Cardiology suggests that patients with chronic stable angina require careful cardiovascular risk assessment before initiating ED treatment 7
  • A cardiovascular risk assessment should be performed before initiating ED treatment to ensure the patient has sufficient cardiac health for sexual activity, categorizing patients as low, intermediate, or high cardiovascular risk, with high-risk patients requiring cardiac condition stabilization before ED treatment, as recommended by The American Heart Association, The Princeton Consensus, and the Mayo Clinic 8, 3, 1
  • Checking for testosterone deficiency (total testosterone <300 ng/dL) is part of the medical evaluation, and considering psychological factors that may contribute to erectile dysfunction and referring for psychosexual counseling if appropriate, as suggested by The American Urological Association 1

Treatment Considerations and Monitoring

  • Absolute contraindication: Concurrent use of nitrates due to risk of severe hypotension, and caution with alpha-blockers is advised, with a recommended wait of at least 24 hours after sildenafil and 48 hours after tadalafil before administering nitrates, as reported by 3, 1, 9
  • The American Heart Association implies that PDE5 inhibitors like tadalafil are safe for patients with stable cardiovascular disease, provided they are not taking nitrates concurrently 8
  • Reassessing treatment response in 4-6 weeks and monitoring for side effects are essential for adjusting the treatment plan, and regular assessment of treatment efficacy and side effects, monitoring for any changes in psychiatric condition or medications, and assessing for any cardiovascular symptoms that may develop is recommended, as reported by 8, 1, 3
  • Erectile dysfunction treatment should be tailored to the individual's needs, considering underlying medical conditions, medications, and psychological factors, as recommended by the American College of Physicians, the American Urological Association, and the British Medical Journal 5, 10, 11
  • Lifestyle modifications, such as weight loss, smoking cessation, physical activity, regular exercise, and a healthy diet, can improve erectile function and sexual function in men with risk factors, as suggested by the National Comprehensive Cancer Network and reported by 12
  • Combination therapy with PDE5i and other therapies, such as testosterone therapy, may be more effective than PDE5i alone, according to the American College of Physicians and the American Urological Association 5, 1
