Steroid Dosing in Lymphoma

Introduction to Lymphoma Treatment

Dosing by Lymphoma Type

  • For Diffuse Large B-Cell Lymphoma (DLBCL), the National Comprehensive Cancer Network recommends prednisona, 100 mg/day by mouth on days 1-5, as a component of the R-CHOP-21 regimen 1
  • For Classical Hodgkin Lymphoma, the European Society for Medical Oncology recommends prednisona, 40 mg/m² by mouth on days 1-14, as part of the BEACOPP escalated regimen 2
  • For Peripheral T-Cell Lymphomas (PTCL), the National Comprehensive Cancer Network recommends prednisona, 100 mg/day by mouth on days 1-5, every 3 weeks, as part of the CAP regimen 4
  • For Hepatosplenic T-Cell Lymphoma, the European Society for Medical Oncology recommends prednisona, 40 mg/m² by mouth on days 1-14, as part of the CHOEP/EPOCH intensified regimen 6

Rescue Regimens

  • The European Society for Medical Oncology recommends dexametasona, 40 mg by mouth or IV on days 1-4, every 3 weeks, as part of the DHAP protocol 3

Critical Dosage Considerations

  • The American Society of Clinical Oncology states that dose reduction for hematologic toxicity is not recommended in patients with curative intent 7
  • The National Comprehensive Cancer Network recommends maintaining dose intensity as critical for optimal outcomes 9

Special Population Adjustments

  • For obese patients, the American College of Hematology recommends using full doses based on weight without arbitrary limits 1
  • For patients over 60 years with Hodgkin lymphoma, the European Society for Medical Oncology recommends avoiding BEACOPP escalated and using ABVD without prednisona 3

Essential Support and Monitoring

  • The National Comprehensive Cancer Network recommends mandatory prophylaxis against Pneumocystis jirovecii with trimethoprim-sulfamethoxazole during treatment and 6-12 months after rituximab 1
  • The American Society of Clinical Oncology recommends antiviral prophylaxis with acyclovir or equivalent 1
  • The National Comprehensive Cancer Network recommends monitoring for hepatitis B reactivation every 1-3 months during and 6-12 months after rituximab 9
