PSA Screening Guidelines for Prostate Cancer

Introduction to Screening

  • The American Cancer Society recommends engaging in shared decision-making about PSA screening for average-risk men at age 50, discussing the small mortality benefit and substantial harms, then proceeding with testing only if the patient expresses a clear preference for screening after understanding these trade-offs 1, 2

Risk Stratification

  • Average-risk men are recommended to begin shared decision-making conversations at age 50 for men expected to live at least 10 years, with the strongest randomized trial evidence supporting screening starting at age 55, showing approximately 1.3 fewer prostate cancer deaths per 1,000 men screened over 13 years 1, 2
  • African American men should start screening at age 45 due to higher incidence and mortality rates 1, 2
  • Men with one first-degree relative diagnosed before age 65 should start screening at age 45 1, 2
  • Men with multiple first-degree relatives diagnosed before age 65 should start screening at age 40 1, 2

Screening Intervals

  • Men with PSA <1.0 ng/mL should repeat screening every 2-4 years 1, 2
  • Men with PSA 1.0-2.5 ng/mL should repeat screening annually to every 2 years 1, 2
  • Men with PSA ≥2.5 ng/mL should screen annually with consideration for further evaluation 2
  • Men with PSA ≥4.0 ng/mL should consider biopsy referral 2

Stopping Screening

  • PSA screening should be discontinued at age 70 in most men, except for very healthy men with minimal comorbidity, prior elevated PSA values, and life expectancy >10-15 years 1, 2

Guideline Recommendations

  • The American Urological Association and the American Cancer Society support earlier initiation of screening (age 45-50) with risk stratification 1, 2
  • The European Society for Medical Oncology (ESMO) guidelines state PSA screening should not be encouraged for all asymptomatic men but well-informed men suitable for screening should have access to PSA testing upon request 4, 5
