Acyclovir Administration Guidelines

Importance of Hydration

  • The American College of Physicians recommends that acyclovir is primarily excreted via the kidneys through glomerular filtration and tubular secretion, making adequate hydration critical during administration 3
  • Adequate hydration is essential to prevent acyclovir-induced nephrotoxicity, with the risk of nephropathy reduced by maintaining adequate hydration and monitoring renal function during acyclovir therapy 1, 2
  • Acyclovir-induced nephrotoxicity occurs in up to 20% of patients, typically manifesting after 4 days of intravenous therapy 2, 4

Administration and Monitoring Recommendations

  • The Infectious Diseases Society of America recommends maintaining adequate hydration before, during, and after acyclovir administration is essential 1, 2
  • Regular monitoring of renal function is essential during acyclovir therapy, with dose adjustment required in patients with pre-existing renal impairment 1, 2
  • For patients with renal dysfunction, the acyclovir dose should be reduced to 200 mg every 12 hours, and for hemodialysis patients, administer 200 mg every 12 hours with the first daily dose after dialysis 5

Special Considerations

  • The Centers for Disease Control and Prevention recommends that in children with viral encephalitis, maintaining adequate hydration while on acyclovir is particularly important 1
  • When treating varicella zoster virus infections, the same principles of hydration apply during intravenous acyclovir administration 6
