Glyceryl Trinitrate Administration Guidelines
Introduction to GTN Administration
- The American Heart Association recommends GTN administration via three primary routes: sublingual tablets or spray for acute angina relief, transdermal patches for chronic prophylaxis, or intravenous infusion for unstable angina or acute coronary syndromes 1, 2
Sublingual Administration for Acute Angina
- For acute anginal attacks, the American College of Cardiology suggests dissolving one sublingual tablet under the tongue or using one spray at the first sign of chest discomfort, with a maximum of 3 doses total, each separated by 5 minutes 3
- The European Society of Cardiology recommends administering one dose 5-10 minutes before activities known to trigger angina, such as exercise or sexual activity 1, 3
Intravenous Administration for Acute Coronary Syndromes
- The American Heart Association indicates IV GTN for patients whose symptoms are not relieved after 3 sublingual doses, or for high-risk unstable angina/NSTEMI patients who are not hypotensive, starting at 10 mcg/min via continuous infusion 1, 2, 4
- The American College of Cardiology suggests increasing the dose by 10 mcg/min every 3-5 minutes until symptom relief or blood pressure response occurs, with a commonly used ceiling of 200 mcg/min 1, 2, 4
Blood Pressure Parameters
- The American Heart Association recommends not using GTN if systolic BP <90 mmHg or >30 mmHg below baseline, and targeting systolic BP: maintain ≥110 mmHg in normotensive patients 2, 4
Tolerance Management
- The European Society of Cardiology states that tolerance develops after 24 hours of continuous infusion and is dose/duration dependent, recommending transition to oral or topical nitrates within 24 hours once patient is stable and symptom-free for 12-24 hours 2, 4
Transdermal Administration for Chronic Prophylaxis
- The American College of Cardiology suggests using transdermal patches (0.2-0.8 mg/h) for sustained nitrate delivery, requiring a nitrate-free interval to prevent tolerance, with application in the morning and removal at bedtime to provide a 10-12 hour nitrate-free interval overnight 1, 5
Absolute Contraindications
- The American Heart Association absolutely contraindicates GTN in patients using phosphodiesterase-5 inhibitors within 24 hours of sildenafil or 48 hours of tadalafil, hypotension, right ventricular infarction, marked bradycardia or tachycardia, and concurrent use with soluble guanylate cyclase stimulators 1, 2, 3, 4
