Fatty Liver Disease Management Recommendations

Introduction to Management

  • Lifestyle modifications, including weight loss of 7-10% through caloric restriction and regular physical activity, are the cornerstone of treatment for all patients with fatty liver disease, according to the American Association for the Study of Liver Diseases 1, 2

Assessment and Diagnosis

  • Perform comprehensive metabolic and serological screening to confirm NAFLD diagnosis and exclude co-existing liver diseases, as recommended by The Lancet Gastroenterology and Hepatology 4
  • Assess fibrosis severity using sequential non-invasive tests, such as FIB-4 followed by ELF, transient elastography, or ARFI, as suggested by The Lancet Gastroenterology and Hepatology 5, 4
  • Conduct a medicines review to identify and potentially discontinue medications that may worsen steatosis, such as corticosteroids, amiodarone, methotrexate, and tamoxifen, according to The Lancet Gastroenterology and Hepatology 5

Lifestyle Modifications

  • Target gradual weight loss of 7-10% of total body weight, as recommended by the American College of Gastroenterology 1
  • Aim for progressive weight loss of less than 1 kg/week to avoid complications from rapid weight loss, according to the Clinical and Molecular Hepatology 1
  • Follow a Mediterranean diet pattern, characterized by reduced carbohydrate intake, increased monounsaturated and omega-3 fatty acid intake, and high consumption of vegetables, fruits, legumes, nuts, seeds, and whole grains, as suggested by the Advances in Nutrition 6
  • Limit excess fructose consumption and avoid processed foods and beverages with added fructose, as recommended by The Lancet Gastroenterology and Hepatology 6, 4
  • Replace saturated fats with polyunsaturated and monounsaturated fats, according to the Advances in Nutrition 6
  • Engage in at least 150-300 minutes of moderate-intensity aerobic exercise per week, as suggested by the Praxis Medical Insights 3

Management of Comorbidities

  • Treat hypertension according to established guidelines, as recommended by The Lancet Gastroenterology and Hepatology 7
  • Manage dyslipidemia with statins in patients at increased cardiovascular risk, according to The Lancet Gastroenterology and Hepatology 7
  • Optimize diabetes management, as diabetes increases the risk of HCC and liver-related complications, as suggested by the Clinical and Molecular Hepatology 2
  • Recommend smoking cessation and alcohol abstinence to reduce HCC risk, according to the Clinical and Molecular Hepatology 1, 2

Monitoring and Follow-up

  • Patients with NAFLD who have significant-advanced fibrosis or cirrhosis should be managed in secondary care, as recommended by The Lancet Gastroenterology and Hepatology 7
  • For patients with cirrhosis, implement HCC surveillance with right upper quadrant ultrasound every 6 months, screening for esophageal varices, and referral to transplant center when appropriate, according to the Mayo Clinic Proceedings 8
  • Regularly reassess fibrosis using non-invasive tests every 1-3 years to monitor treatment response or disease progression, as suggested by The Lancet Gastroenterology and Hepatology 5
