Clinical Indications of Endophthalmitis

Risk Factors and Clinical Presentations

  • The American Academy of Ophthalmology suggests that recent ocular surgery, particularly cataract surgery, is a significant risk factor for endophthalmitis, with coagulase-negative staphylococci being the most common causative organism 2
  • Penetrating ocular trauma with environmental organisms like Bacillus cereus, Pseudomonas aeruginosa, or environmental mycobacteria is also a risk factor for endophthalmitis 2
  • Corneal transplantation is a rare but devastating complication that can lead to endophthalmitis, as reported by the American Academy of Ophthalmology 3
  • The Infectious Diseases Society of America recommends a dilated retinal examination within the first week of therapy for all candidemia patients to detect active candidemia, a risk factor for endophthalmitis 4
  • Injection drug use with hematogenous dissemination of organisms is a risk factor for endophthalmitis, according to the Infectious Diseases Society of America 6

Diagnostic Approach

  • The American Academy of Ophthalmology recommends direct ophthalmoscopic examination as a required step for definitive clinical diagnosis of endophthalmitis 1
  • Culture of vitreous or aqueous humor via diagnostic vitreous tap before initiating treatment is necessary to identify the causative organism, as suggested by the American Academy of Ophthalmology 1

Organism-Specific Patterns

  • Coagulase-negative staphylococci (68.4%) and Streptococcus species (8.2%) are the most common causative organisms in post-surgical endophthalmitis, as reported by the American Academy of Ophthalmology 7
  • Propionibacterium acnes should not be dismissed as a contaminant in chronic post-operative cases, as it is a true pathogen in this setting, according to the Infectious Diseases Society of America 2
  • Candida species, particularly C. albicans, are common causative organisms in fungal endophthalmitis, especially in hospitalized patients with candidemia, as reported by the Infectious Diseases Society of America 4
  • Aspergillus species are common causative organisms in fungal endophthalmitis, especially in immunocompromised patients, injection drug users, or those with disseminated aspergillosis, according to the Infectious Diseases Society of America 6

Prognostic Indicators

  • Direct macular involvement is a poor prognostic indicator for visual acuity recovery, as reported by the American Academy of Ophthalmology 1
