Management of Diverticulitis with Osmotic Laxatives

Safety and Efficacy

  • The American Gastroenterological Association suggests that osmotic laxatives, such as MiraLAX, can be safely used in patients with diverticulitis, as there is no contraindication in current major gastroenterology guidelines 1, 2, 3, 4
  • MiraLAX works gently by drawing water into the colon without causing forceful contractions that could worsen inflammation, making it a suitable option for patients with diverticulitis 4
  • The medication does not increase intra-colonic pressure significantly, which is the primary mechanical concern in diverticulitis 5

Clinical Considerations

  • Maintaining soft, regular stools with MiraLAX may reduce straining and intra-colonic pressure in patients with diverticulitis 4
  • Fiber supplementation is recommended after acute diverticulitis resolves to prevent recurrence, and MiraLAX can serve as a bridge therapy or adjunct if patients cannot tolerate adequate dietary fiber initially 4, 5
  • A high-fiber diet (or fiber supplementation) is the preferred long-term approach over chronic osmotic laxative use 5

Contraindications and Precautions

  • NSAIDs (non-aspirin) should be avoided as they moderately increase the risk of diverticulitis episodes and complications 1, 2
  • Stimulant laxatives may be less ideal than osmotic agents for patients with diverticulitis, although no direct evidence addresses this 4
