Management of HOCM with Mitral Regurgitation and Infective Endocarditis
Diagnostic Approach
- The European Society of Cardiology recommends transthoracic echocardiography (TTE) as the initial diagnostic test in all suspected cases of IE, with a sensitivity of 40-70% and specificity around 90% 3, 4
- Transesophageal echocardiography (TEE) is mandatory when initial TTE is negative but clinical suspicion remains high, with a sensitivity of 90-100% 3, 5
Management Approach
- The European Heart Association indicates that surgery is urgently required (within a few days) for patients with HOCM and IE with heart failure due to severe mitral regurgitation, large vegetation (>10 mm) on the mitral valve, or locally uncontrolled infection 1, 6, 7
- Emergency surgery (within 24 hours) is necessary for severe acute regurgitation causing refractory pulmonary edema or cardiogenic shock, or vegetation with evidence of ongoing embolization despite appropriate antibiotics 1, 3, 6
Antimicrobial Therapy
- The European Heart Association recommends that appropriate antibiotic therapy should be initiated immediately after blood cultures are obtained, with a treatment duration of typically 4-6 weeks 1, 6
- Persistent fever and positive blood cultures after 7-10 days of appropriate antibiotic therapy may indicate uncontrolled infection and necessitate surgical intervention 2
Monitoring and Follow-up
- Repeat echocardiography is essential when clinical features suggest new development of intracardiac complications, unexplained progression of heart failure symptoms, or new cardiac murmurs or conduction abnormalities develop 5
- Monitoring for vegetation size changes is important, as increasing vegetation size under therapy indicates higher risk of embolic events 2
Prognosis and Complications
- Heart failure is the most frequent complication of IE and represents the most common indication for surgery, according to the European Heart Association 1, 6
Pitfalls and Caveats
- Delayed diagnosis and treatment significantly impact clinical outcomes, as stated by the European Heart Association 8
