Management of Measles in Children
Immediate Actions: Isolation and Reporting
- The Centers for Disease Control and Prevention recommends isolating the patient immediately for at least 4 days after rash onset, as the child remains contagious from 4 days before through 4 days after rash appearance 3
- The CDC advises contacting the local or state health department immediately, as one confirmed case of measles constitutes an urgent public health situation requiring prompt investigation 1, 2, 4
Diagnostic Confirmation
- The World Health Organization recommends collecting blood for measles-specific IgM antibody testing during the first clinical encounter, even before laboratory results return, as control activities should not be delayed 1, 2, 4
- The CDC notes that if IgM is negative within the first 72 hours of rash onset, a second specimen should be obtained at least 72 hours after rash onset, as IgM may not be detectable early 5, 4
- The WHO states that IgM peaks approximately 10 days after rash onset and remains detectable for at least 1 month 2
Essential Treatment: Vitamin A Supplementation
- The American Academy of Pediatrics recommends administering vitamin A supplementation to all children with clinical measles, as this is the only evidence-based intervention proven to reduce measles mortality 3, 5
- The WHO advises dosing protocol as follows: children ≥12 months should receive 200,000 IU orally on day 1, and children <12 months should receive 100,000 IU orally on day 1 3, 5, 4
Supportive Care and Complication Management
- The CDC recommends monitoring for and treating complications with standard therapies, including bacterial superinfections, diarrhea, and acute lower respiratory infections 5, 4, 3
Special Populations Requiring Enhanced Management
- The CDC advises that immunocompromised patients should receive immune globulin (IG) 0.5 mL/kg (maximum 15 mL) if exposed, regardless of vaccination status 3, 5
- The WHO recommends that pregnant women should receive IG 0.25 mL/kg (maximum 15 mL) within 6 days of exposure 3
