Anxiety Disorders
Diagnosis and Characteristics
- Social anxiety disorder is characterized by marked and persistent fear of social situations, fear that is disproportionate to the actual threat, avoidance of feared situations, and significant interference with normal functioning, with symptoms typically persisting for 6 months or more, affecting individuals with a median age of 13 years 1
- Approximately one-third of patients have comorbid psychiatric disorders, including other anxiety disorders and depression, as reported by the National Institute of Mental Health and the American Academy of Child and Adolescent Psychiatry 1
Treatment
- The American College of Neuropsychopharmacology and the American Psychiatric Association recommend Individual Cognitive Behavioral Therapy (CBT) specifically developed for social anxiety disorder, with individual therapy preferred over group therapy due to superior clinical and economic effectiveness 1
- The American College of Physicians recommends SSRIs (e.g., sertraline, escitalopram, paroxetine, and fluvoxamine) as first-line pharmacological treatment for anxiety disorders, citing high efficacy, relatively safe side effect profile, and low risk of dependence 2, 3
- Venlafaxine, a Serotonin-Norepinephrine Reuptake Inhibitor (SNRI), is an alternative first-line option, starting at 20 mg once daily, titrating to 20-50 mg daily for social anxiety disorder, as recommended by the American College of Neuropsychopharmacology and the American Psychiatric Association 1
- Duloxetine is the only SNRI with an FDA indication specifically for the treatment of anxiety disorders (generalized anxiety disorder), as recommended by the American College of Physicians 3
Desvenlafaxine dosing is as follows:
| Condition | Initial Dose | Titration Range | | --- | --- | --- | | Major Depressive Disorder | 20 mg once daily | 40-50 mg daily | | Social Anxiety Disorder | 20 mg once daily | 20-50 mg daily | | Obsessive-Compulsive Disorder | 20 mg once daily | 40-50 mg daily | 1
- Common SNRI side effects include diaphoresis, dry mouth, abdominal discomfort, nausea, vomiting, diarrhea, dizziness, headache, tremor, insomnia, somnolence, decreased appetite, and weight loss, while serious but uncommon effects include suicidal thinking and behavior, behavioral activation/agitation, hypomania, mania, sexual dysfunction, seizures, abnormal bleeding, and serotonin syndrome 3
- For young individuals aged 7 to 17 years, selective serotonin reuptake inhibitors (SSRIs) are recommended as a first-line treatment, according to the American Academy of Child and Adolescent Psychiatry, with monitoring for suicidal ideation 3
- Elderly patients should start on lower doses of medications and titrate more slowly, while patients with hepatic or renal impairment require dose adjustments and careful monitoring, and pregnancy and lactation require special consideration, with SSRIs used with caution when benefits outweigh risks 1, 3
- Lifestyle modifications, such as establishing a consistent morning routine, regular exercise, good sleep hygiene, limiting screen time before bed and after waking, spending time in nature, and practicing mindfulness and meditation, can be beneficial for patients with anxiety disorders, as recommended by the American College of Neuropsychopharmacology 1, 4
- Treatment response should be assessed after 4-6 weeks of medication, and effective medication should be continued for at least 6-12 months after symptom remission, tapering gradually when discontinuing to avoid withdrawal symptoms, and considering tapering medications if symptoms are under control and primary sources of anxiety are no longer present 1, 5
- The treatment plan should be adjusted based on response, with consideration of increasing the dose of antidepressants, intensifying CBT, or adding other medications such as pregabalin or quetiapine, and comorbidities may require additional targeted interventions, with treatment resistance prompting reevaluation of diagnosis, medication adherence, and consideration of alternative or augmentation strategies 1
