Treatment of Residual Diffuse IDH-Mutant Astrocytoma of the Insula

Introduction to Treatment Guidelines

  • The American College of Radiology and other oncology guideline societies recommend radiotherapy (50-54 Gy in 1.8-2 Gy fractions) followed by PCV chemotherapy for residual diffuse IDH-mutant astrocytoma following incomplete resection, based on RTOG 9802 trial data showing overall survival prolongation from 7.8 to 13.3 years 1, 2

Treatment Algorithm Based on Grade and Patient Factors

  • For WHO Grade 2 IDH-Mutant Astrocytoma with residual disease, the National Comprehensive Cancer Network recommends radiotherapy (50-54 Gy in 1.8-2 Gy fractions) followed by adjuvant PCV chemotherapy, specifically indicated for patients with incomplete resection and/or age >40 years 1, 2, 3, 4
  • Temozolomide chemoradiotherapy is an acceptable alternative when PCV toxicity is a concern, though evidence is less robust, as stated by the European Association of Neuro-Oncology 1, 4

Critical Considerations for the Insula Location

  • The American Association of Neurological Surgeons suggests that second surgery should always be considered first before proceeding to adjuvant therapy, even for insular tumors, though clinical benefit may be limited to cases where gross total resection can be achieved 8, 9
  • Maximal safe resection remains the best initial therapeutic approach, and re-resection should be evaluated before committing to radiotherapy, according to the Congress of Neurological Surgeons 1, 2

Important Caveats and Pitfalls

  • The National Cancer Institute advises against premature radiotherapy in young patients with complete resection, as early radiotherapy prolongs progression-free survival but has NOT been shown to improve overall survival compared to radiotherapy at progression 2, 3, 5
  • The European Society for Medical Oncology recommends not using watch-and-wait for residual disease, as observation strategies should only be considered in exceptional situations and specifically after gross total resection in younger patients, NOT with residual tumor 1, 2

Treatment Sequence

  • The American Society for Radiation Oncology recommends re-evaluating surgical options for additional resection of residual disease, and if further resection is not feasible, initiating radiotherapy within 12 weeks of diagnosis, with dosing of 50-54 Gy for grade 2 and 54-60 Gy for grade 3, delivered in 1.8-2 Gy fractions 1, 3, 7, 8, 9
  • Adjuvant chemotherapy should be PCV for grade 2 (or temozolomide if toxicity concern) and temozolomide for grade 3, as recommended by the Society for Neuro-Oncology 1, 4, 8

At Progression or Recurrence

  • The European Association of Neuro-Oncology recommends temozolomide (or nitrosourea) as the standard chemotherapy at progression after initial surgery and radiotherapy, with treatment choice depending on neurological status, patterns of progression, and first-line therapy received 1, 3, 6, 8
