Ipratropium-Albuterol Combination Nebulizer for Infants

Introduction to Treatment Guidelines

  • The National Asthma Education and Prevention Program (NAEPP) guidelines state that dosages are only provided for products with FDA approval or sufficient clinical trial data in appropriate age ranges, and ipratropium-albuterol combination nebulizer is not recommended for routine use in infants under 12 months due to lack of established safety and efficacy 1, 2, 3
  • The National Asthma Education and Prevention Program (NAEPP) guidelines explicitly state that dosages are only provided for products with FDA approval or sufficient clinical trial data in appropriate age ranges, and the guideline tables list "0.5 mg/3 mL" for the "<5 Years" category but provide no specific dosing for infants under 12 months 1, 2, 3
  • The combination product contains EDTA and is formulated for older children, not suitable for infants under 12 months 1, 2, 3

Alternative Approach for Infants Under 12 Months

  • Albuterol nebulizer solution at a dose of 0.63 mg/3 mL is recommended for children under 5 years, including infants, as first-line therapy for bronchodilator treatment 1, 2, 3
  • Albuterol can be administered every 4-6 hours as needed, or more frequently (every 20 minutes for 3 doses) during acute exacerbations 4
  • Weight-based dosing of 0.15 mg/kg (minimum 2.5 mg) may be used for severe exacerbations 4

Clinical Decision Algorithm

  • For a 5-month-old with bronchospasm, start with albuterol 0.63 mg nebulized every 4-6 hours as needed 1, 2, 3
  • If inadequate response after 15-30 minutes, consider adding ipratropium bromide alone (100-125 mcg) to albuterol in the nebulizer, but not the pre-mixed ipratropium-albuterol combination product (DuoNeb) 5
  • Do not use the pre-mixed ipratropium-albuterol combination product (DuoNeb) in this age group due to high ipratropium dose 1, 2, 3

Important Safety Considerations

  • The fixed-dose combination delivers 0.5 mg ipratropium, which is 4-5 times higher than the recommended maximum for very young children, and can cause increased wheezing in some individuals and drying of respiratory secretions 1, 5
  • Ipratropium can cause side effects, and approximately 40% of young children with recurrent airway obstruction obtain benefit from ipratropium, meaning many will not respond 1, 2, 3
