Management of Hyperphosphatemia with Low Intact PTH in CKD

Understanding the Clinical Picture

  • The American Journal of Kidney Diseases recommends immediately suspecting and managing adynamic bone disease by withholding or reducing vitamin D therapy and calcium-based phosphate binders, while aggressively treating hyperphosphatemia with non-calcium-based phosphate binders to allow PTH levels to rise and restore bone turnover in CKD patients with hyperphosphatemia and low intact PTH 1
  • Adynamic bone disease is a low-turnover bone disorder where PTH levels below 100 pg/mL in Stage 5 CKD indicate suppressed bone formation, according to the American Journal of Kidney Diseases 1
  • Hyperphosphatemia with low PTH suggests oversuppression of parathyroid function from excessive vitamin D therapy or calcium-based phosphate binders, as stated in the American Journal of Kidney Diseases 1
  • Increased cardiovascular risk is associated with both the low-turnover bone state and hyperphosphatemia, which independently promote vascular calcification, as reported in the American Journal of Kidney Diseases 2

Immediate Management Algorithm

  • The American Journal of Kidney Diseases recommends stopping or significantly reducing vitamin D sterols immediately to permit PTH levels to increase above 100 pg/mL and restore bone turnover in CKD patients with hyperphosphatemia and low intact PTH 1
  • Eliminating or minimizing calcium-based phosphate binders is recommended to allow PTH to rise, according to the American Journal of Kidney Diseases 1
  • Implementing strict dietary phosphate restriction to 800-1,000 mg/day is critical as hyperphosphatemia drives vascular calcification even with low PTH, as stated in the American Journal of Kidney Diseases 2

Target Serum Phosphorus Levels

  • The American Journal of Kidney Diseases recommends targeting serum phosphorus levels of 3.5-5.5 mg/dL (1.13-1.78 mmol/L) for Stage 5 CKD (dialysis) patients and maintaining ≤4.6 mg/dL (1.48 mmol/L) for Stage 3-4 CKD patients 2

Monitoring Protocol

  • The American Journal of Kidney Diseases recommends checking serum calcium and phosphorus every 2 weeks initially, then monthly once stable, and measuring intact PTH monthly until levels rise above 100 pg/mL, then every 3 months 3
  • Target PTH range for Stage 5 CKD is 150-300 pg/mL once bone turnover is restored, as stated in the American Journal of Kidney Diseases 3

Critical Pitfalls to Avoid

  • Avoiding aluminum-containing phosphate binders is recommended as aluminum can cause osteomalacia and worsen low-turnover bone disease, according to the American Journal of Kidney Diseases 1
  • Never attempting to normalize PTH to levels seen in patients without CKD is recommended as this leads to or perpetuates adynamic bone disease, as stated in the American Journal of Kidney Diseases 5
