Pituitary Apoplexy Management
Clinical Presentation
- Patients with pituitary apoplexy typically present with pupil-involving third nerve palsy, characterized by a dilated pupil, if the oculomotor nerve is affected, according to the American Academy of Ophthalmology 1
- Pituitary apoplexy occurs more frequently in men aged 50-60 years, but can develop at any age, including during pregnancy or postpartum periods, and is rare in children and adolescents, but can be more severe when it occurs in pediatric populations 2
Neuroimaging and Diagnosis
- Emergent brain MRI with gadolinium should be performed, with attention to the pituitary and brainstem, as recommended by the American Academy of Ophthalmology 1
- MRI is the gold standard for preoperative assessment of pituitary adenomas, according to the Congress of Neurological Surgeons 3
- Comprehensive preoperative assessment is recommended for patients with pituitary lesions, as suggested by the American College of Radiology and the Congress of Neurological Surgeons 4, 3
- Measurement of serum sodium and electrolytes is essential in the diagnosis of diabetes insipidus, as it can help identify electrolyte imbalances 5
- Pituitary hormone assessment, including morning cortisol and ACTH, TSH and free T4, gonadotropins, and prolactin, is crucial in evaluating patients with suspected hypopituitarism 5
Treatment and Management
- Urgent transsphenoidal surgery is recommended for patients with severe or progressive visual impairment, altered consciousness, or neurological deterioration, preferably performed within seven days of symptom onset, and using an endoscopic approach when available, as recommended by the Congress of Neurological Surgeons and the American Association of Neurological Surgeons 2
- The American College of Oncology recommends immediate initiation of intravenous methylprednisolone 1 mg/kg after obtaining blood samples for pituitary axis assessment, with subsequent treatment stratified based on symptom severity, for patients with severe headache, visual disturbances, altered consciousness, hypotension, or severe electrolyte disturbances 6
- Oral prednisolone 0.5-1 mg/kg once daily can be used for patients with headache without visual disturbance, fatigue/mood alteration, but who are hemodynamically stable, with no electrolyte disturbance, according to the American College of Oncology 6
- The American College of Clinical Endocrinologists recommends that cortisol replacement be initiated before thyroid replacement to avoid precipitating adrenal crisis in patients with hypopituitarism 7
Long-term Care and Follow-up
- Hormone deficiencies resulting from structural damage to the pituitary are typically permanent and require lifelong replacement therapy, according to the American Association of Clinical Endocrinologists 7
- Patients with immune checkpoint inhibitor-related hypophysitis may develop multiple pituitary hormone deficiencies, including central hypothyroidism and adrenal insufficiency, which require lifelong replacement therapy 7
- Regular assessment of pituitary function and careful monitoring for water and electrolyte disturbances are crucial, due to the high risk of diabetes insipidus (26%) and SIADH (14%), as recommended by the Endocrine Society 2
- Long-term follow-up by a multidisciplinary team is essential, as recommended by the Endocrine Society 2
- Endocrinology consultation and regular monitoring of thyroid function tests are recommended, as stated by the American College of Oncology and the Endocrine Society 6, 2
Patient Education and Support
- Patients should be educated on "sick day rules" for steroid adjustment during illness, and prescribed and educated on the use of IM steroid if required, as recommended by the American Medical Association 6, 5
- Consideration of a medical alert card or bracelet is recommended, especially for patients with adrenal insufficiency, as recommended by the American Medical Association 6, 5
- The Endocrine Society recommends that all patients with adrenal insufficiency receive education on "sick day rules" and carry medical alert identification to ensure prompt treatment in case of an emergency 8
- Formal visual field assessment should be performed, and patients with abnormal results should inform their driver licensing agency, according to the American College of Oncology 6
