Management of Severe Neutropenia with Filgrastim

Dosing Recommendation

  • The National Comprehensive Cancer Network recommends administering Zarxio (filgrastim) at 5 μg/kg/day subcutaneously until the ANC recovers to at least 1,000/mm³ 1
  • Continue daily administration of filgrastim until ANC recovers to at least 1,000/mm³, as recommended by the Journal for ImmunoTherapy of Cancer 2, 3
  • Discontinue filgrastim administration once ANC recovers to >1,000/mm³, as recommended by the Journal for ImmunoTherapy of Cancer 2, 3

Rationale for Treatment

  • Patients with severe neutropenia, such as an ANC of 0.37 × 10⁹/L, are at high risk for infection, according to the Annals of Oncology 4
  • Filgrastim is strongly recommended for patients with chemotherapy-induced neutropenia to reduce the risk of infection and potentially shorten hospitalization, as stated by the Journal for ImmunoTherapy of Cancer 2

Important Precautions

  • The Annals of Oncology contraindicates filgrastim administration if the patient is receiving radiation therapy to the chest due to increased risk of complications 4
  • Monitor for potential side effects including bone pain, which is common but usually diminishes within the first few weeks of treatment, as noted by the Journal of the National Comprehensive Cancer Network 1

Follow-up Recommendations

  • Consider antibiotic prophylaxis if neutropenia is expected to be prolonged, as recommended by the Journal for ImmunoTherapy of Cancer 2

Filgrastim Administration in Neutropenia

Protocolo de Dosificación y Duración

  • The American Society of Clinical Oncology (ASCO) guideline recommends initiating filgrastim 24-72 hours after completing myelosuppressive chemotherapy, with a recommended dose of 5 μg/kg/day subcutaneously 5, 6
  • The objective is to achieve an absolute neutrophil count (ANC) of 2-3 × 10⁹/L (2,000-3,000/mm³), as specified by the ASCO guideline, although it is not necessary to achieve an ANC >10 × 10⁹/L 6, 7

Contraindicaciones Durante el Tratamiento

  • The European Society for Medical Oncology (ESMO) warns against administering filgrastim during thoracic radiotherapy due to increased complications and mortality 5, 8
  • There is a risk of severe thrombocytopenia if filgrastim is administered immediately before or simultaneously with chemotherapy 5, 8

Filgrastim for Chemotherapy-Induced Neutropenia

Primary Prophylaxis

  • The National Comprehensive Cancer Network recommends filgrastim initiation 1-3 days after completion of chemotherapy at a dose of 5 mcg/kg/day subcutaneously, continued daily until post-nadir ANC recovery to 2,000-3,000/mm³, and should NOT be given on the same day as chemotherapy 9, 10
  • Filgrastim should be administered subcutaneously, with a standard dose of 5 mcg/kg/day for all clinical settings except stem cell mobilization, starting 24-72 hours after last chemotherapy dose 9, 10
  • The duration of filgrastim treatment should continue until ANC reaches 2,000-3,000/mm³, as achieving ANC >10,000/mm³ is unnecessary and should be avoided 10, 11

Alternative Prophylaxis

  • Pegfilgrastim is considered equally effective to 10-11 daily filgrastim injections and is a Category 1 recommendation alongside filgrastim, with a single 6 mg dose subcutaneously administered once per cycle, 24 hours after chemotherapy completion 9, 10, 11
  • The 6 mg formulation of pegfilgrastim should not be used in patients weighing <45 kg 10

Contraindications and Precautions

  • The American Society of Clinical Oncology and the National Comprehensive Cancer Network recommend that filgrastim is absolutely contraindicated in patients receiving concurrent chest/thoracic radiotherapy due to increased complications and mortality 11
  • Filgrastim administration within 24 hours before or simultaneously with chemotherapy is contraindicated due to increased risk of severe thrombocytopenia and febrile neutropenia 9, 11

Special Populations

  • The American Society of Clinical Oncology recommends that routine use of filgrastim in pediatric Acute Myeloid Leukemia (AML)/Acute Lymphoblastic Leukemia (ALL) is NOT recommended due to theoretical concerns about stimulating leukemic blast growth 10
  • In allogeneic transplant patients, filgrastim may safely postpone initiation until day 5-7 post-transplant, with clinical benefit restricted to ANC recovery only 11
