DaT Scan Diagnostic Criteria and Clinical Applications
Technical and Diagnostic Considerations
- A normal DaT scan shows symmetric, comma-shaped uptake in both the caudate nuclei and putamina bilaterally, whereas an abnormal scan demonstrates loss of the normal comma shape, indicating reduced dopamine transporter density from neuronal degeneration 1
- Reduced striatal uptake has 78% sensitivity and 90% specificity for differentiating dementia with Lewy bodies (DLB) from Alzheimer's disease 1
Dementia Syndromes Diagnosis
- The American College of Radiology suggests that DaT scanning is a suggestive feature for diagnosing dementia with Lewy bodies (DLB) when clinical features overlap with Alzheimer's disease, with reduced striatal uptake indicating DLB pathology 2, 1
- DaT imaging can facilitate earlier diagnosis in patients with prodromal DLB symptoms, and helps identify patients with coexistent Alzheimer's disease and DLB pathology 1, 2, 3
Diagnostic Value and Limitations
- The American College of Radiology recommends FDG-PET before DaT scanning for dementia evaluation due to cost considerations and higher diagnostic yield, although DaT scanning can provide diagnostic value in certain scenarios 5
- DaT scanning should be regarded as supportive rather than diagnostic of dopamine-deficient parkinsonism, and cannot differentiate between different causes of dopaminergic degeneration (Parkinson's disease vs. atypical parkinsonisms) 5
