Side Effects of Zepbound (Tirzepatide)

Gastrointestinal Side Effects

  • Zepbound causes predominantly gastrointestinal side effects, including nausea, diarrhea, and constipation, that are typically dose-dependent and most pronounced during dose initiation and escalation, according to the American College of Clinical Endocrinologists 1
  • Abdominal pain, dyspepsia, and gastroesophageal reflux can also occur, primarily related to delayed gastric emptying caused by GLP-1 receptor activation, which inhibits gastric peristalsis while increasing pyloric tone, as reported by the American Gastroenterological Association 1, 2

Serious Gastrointestinal Complications

  • Severe gastrointestinal complications, such as pancreatitis, require immediate attention and discontinuation of Zepbound, as recommended by the American Diabetes Association 3, 2
  • Gallbladder disorders, including cholelithiasis and cholecystitis, can occur but are usually asymptomatic, with rates ≤1% across all doses, according to the National Institute of Diabetes and Digestive and Kidney Diseases 1

Endocrine and Metabolic Side Effects

  • The risk of thyroid C-cell tumors has been observed in rodents, and Zepbound is contraindicated in patients with personal or family history of medullary thyroid carcinoma or multiple endocrine neoplasia syndrome type 2, as stated by the Endocrine Society 3, 1
  • Hypoglycemia risk increases when used with insulin or insulin secretagogues, occurring in 0.2-1.7% of patients on tirzepatide monotherapy, according to the American Association of Clinical Endocrinologists 1

Cardiovascular Side Effects

  • Increased heart rate may occur and requires monitoring; if cardiac arrhythmia or tachycardia becomes symptomatic, consider beta blockers, as recommended by the American Heart Association 1

Other Notable Side Effects

  • Alopecia (hair loss) has been reported, as noted by the American Academy of Dermatology 1
  • Injection site reactions may occur but are typically mild, according to the Infectious Diseases Society of America 1
  • Acute kidney injury risk exists due to dehydration from gastrointestinal side effects; use caution when initiating or increasing doses in patients with kidney disease, as advised by the National Kidney Foundation 3

Medication Interactions and Special Considerations

  • Delayed gastric emptying creates important clinical implications, including delayed absorption of oral medications, particularly affecting medications with narrow therapeutic index, as reported by the American College of Clinical Pharmacy 2
  • Increased risk of pulmonary aspiration during anesthesia requires consideration of timing relative to surgical procedures, according to the American Society of Anesthesiologists 1
  • Oral contraceptive efficacy may be reduced; patients should switch to non-oral contraceptive methods or add barrier methods for 4 weeks after initiation and after each dose escalation, as recommended by the American College of Obstetricians and Gynecologists 3

Risk Mitigation Strategies

  • To minimize side effects, implement gradual dose titration, starting at the lowest dose and slowly increasing over several weeks, as advised by the American Diabetes Association 2
  • Reduce meal size and avoid high-fat foods, limit alcohol and carbonated drinks, ensure adequate hydration to prevent constipation, and avoid use in patients with gastroparesis, as recommended by the National Institute of Diabetes and Digestive and Kidney Diseases 2, 1
