Management of Melanoma In Situ

Introduction to Margin Recommendations

  • The British Journal of Dermatology guidelines recommend 2-5 mm clinical margins to achieve complete histological excision for melanoma in situ, with the goal of excising the lesion completely with a clear histological margin 1
  • The National Comprehensive Cancer Network recommends a measured margin of 0.5 cm around the visible lesion for standard melanoma in situ, though for large in situ lentigo maligna melanoma, surgical margins greater than 0.5 cm may be necessary to achieve histologically negative margins 2, 3

Site-Specific Considerations

  • For head and neck locations, wider margins (>0.5 cm) are often required due to subclinical extension, with approximately 50% of patients with lentigo maligna requiring margins >0.5 cm 4
  • Extremities and trunk may use 1-cm margins due to ease of cosmetic closure and higher likelihood of failure with 0.5-cm margins 4

Special Considerations for Subtypes

  • Lentigo maligna has a higher risk of subclinical extension, requiring wider margins, and may extend several centimeters beyond visible margins 2, 4
  • Mohs micrographic surgery has a 95.1% clearance rate for melanoma in situ in cosmetically sensitive areas, particularly useful for large or ill-defined lesions on the head and neck 4
  • Topical imiquimod has emerged as a treatment option, especially for lentigo maligna 2, 5
  • Radiotherapy is also used selectively with a recommended target margin of at least 10 mm around the visible lesion 2, 5

Histological Assessment and Margin Width

  • Inadequate histological assessment can lead to underestimating subclinical extension, emphasizing the need for complete histological examination of margins 1
  • Assuming one size fits all is inappropriate, as location, subtype, and size all influence required margin width 4
  • For large lentigo maligna, consider staged excision techniques, perimeter excision, or contoured excision techniques to ensure complete removal 4
  • Missing foci of invasive melanoma can occur if the central excision specimen is not subject to permanent section analysis 4
Location Recommended Margin
Head and Neck >0.5 cm
Extremities and Trunk 1 cm
Lentigo Maligna >0.5 cm
