Duration of Plavix (Clopidogrel) After Heart Attack

Treatment Duration Based on Heart Attack Management

  • The American College of Cardiology recommends a minimum duration of 12 months of dual antiplatelet therapy (DAPT) with clopidogrel 75 mg daily plus aspirin 81 mg daily after either bare-metal stent (BMS) or drug-eluting stent (DES) placement, based on strong evidence from the CURE, PLATO, and TRITON-TIMI 38 trials 1, 2, 3, 4
  • The American College of Cardiology gives a Class I recommendation for a minimum duration of 12 months of clopidogrel 75 mg daily plus aspirin 81 mg daily for patients who received medical therapy alone, based on the CURE trial, which showed a 2.1% absolute reduction in death, heart attack, and stroke 1, 4
  • The ideal duration of clopidogrel plus aspirin for patients who received fibrinolytic therapy is 12 months, based on the CLARITY-TIMI 28 and COMMIT trials, which demonstrated improved outcomes with clopidogrel in STEMI patients receiving fibrinolytics 1, 2, 3, 5

Extending Beyond 12 Months: When to Consider

  • The American College of Cardiology recommends that patients may reasonably continue clopidogrel beyond 12 months if they meet certain criteria, including tolerating DAPT without bleeding complications, not being at high bleeding risk, and understanding the benefits and risks of extended therapy, based on evidence from the PEGASUS-TIMI 54 trial 1, 5, 4
  • Extended DAPT reduces heart attacks by 1-3% but increases bleeding by approximately 1% absolute risk, according to the American College of Cardiology 5, 4

Critical Safety Considerations

  • The American College of Cardiology recommends using low-dose aspirin 81 mg daily (range 75-100 mg) when taking clopidogrel, as higher aspirin doses increase bleeding risk without improving efficacy 1, 2
  • Elective surgery should be delayed for 30 days after bare-metal stent placement and optimally 6 months after drug-eluting stent placement, according to the American College of Cardiology 1, 2, 3

Alternative P2Y12 Inhibitors

  • The American College of Cardiology notes that ticagrelor or prasugrel may be preferred over clopidogrel in certain situations, including maintenance therapy in ACS patients, based on evidence from the PLATO trial 1, 2
  • Ticagrelor is reasonable to use in preference to clopidogrel for maintenance therapy in ACS patients, according to the American College of Cardiology 1
  • Prasugrel is reasonable over clopidogrel in ACS patients who are not at high bleeding risk and have no history of stroke or TIA, based on evidence from the TRITON-TIMI 38 trial 1
