Optimizing Ferrous Sulfate and Levothyroxine Administration

Medication Administration and Interaction

  • For optimal iron absorption, take ferrous sulfate in 2-3 divided doses providing a total of 200 mg elemental iron daily, with standard ferrous sulfate tablets (325 mg) containing approximately 65 mg of elemental iron 1, 2
  • Avoid taking iron with tea, coffee, calcium-containing foods, or fiber-rich foods, which can reduce absorption 3
  • If experiencing significant gastrointestinal side effects, consider taking iron with small amounts of food, starting with a lower dose and gradually increasing to the target dose, taking iron at bedtime, or trying a different iron formulation such as ferrous gluconate or ferrous fumarate 3, 4

Strategies to Minimize Medication Interaction

  • Avoid simultaneous administration of levothyroxine and ferrous sulfate, as this can reduce absorption of both medications, and consider separating their administration by at least 4 hours 2
  • Don't discontinue iron therapy prematurely; continue supplementation until iron stores are fully replenished 4
