Exercise‑Induced Hemodynamic Abnormalities in Early HFpEF

Pathophysiology and Diagnostic Criteria

  • In early‑stage HFpEF, left‑ventricular filling pressures are often normal at rest (LVEDP or PCWP < 15 mm Hg) but rise markedly during exercise, with exercise PCWP ≥ 25 mm Hg meeting the diagnostic threshold for HFpEF. [@1]
  • An exercise pulmonary‑artery systolic pressure ≥ 45 mm Hg provides 96 % sensitivity and 95 % specificity for identifying HFpEF. [@1]
  • The 2022 AHA/ACC/HFSA guidelines state that a diagnosis of HFmrEF or HFpEF requires evidence of elevated LV filling pressures at rest, during exercise, or with other provocations. [@2][@3]

Patient Populations in Which Resting Hemodynamics May Be Normal

  • Older women with hypertension, obesity, diabetes, chronic kidney disease, or atrial fibrillation can have completely normal resting hemodynamics despite symptomatic HFpEF. [@2][@3]

H2FPEF Scoring System (ACC/AHA 2022)

  • The score incorporates six variables: obesity, atrial fibrillation, age > 60 years, use of ≥ 2 antihypertensive agents, echocardiographic E/e′ > 9, and estimated pulmonary‑artery systolic pressure > 35 mm Hg. [@2][@3]
  • Interpretation:
    • Score 0‑1 – low probability; pursue alternative diagnoses. [@2][@3]
    • Score 2‑5 – intermediate probability; exercise echocardiography or invasive exercise hemodynamics is required to confirm or exclude HFpEF. [@2][@3]
    • Score ≥ 6 – high probability (>95 % likelihood) of HFpEF. [@2][@3]

Natriuretic Peptide Considerations

  • Normal or low‑normal BNP/NT‑proBNP levels do not rule out HFpEF, especially when resting filling pressures are normal. [@1][@5]
  • The 2009 ACC/AHA guideline notes that a normal BNP together with normal diastolic filling parameters makes HF less likely, but HF remains a clinical diagnosis and cannot be excluded on this basis alone. [@5]

Exercise Testing Thresholds for Confirmation

  • Exercise PCWP ≥ 25 mm Hg confirms HFpEF. [@1]
  • Exercise pulmonary‑artery systolic pressure ≥ 45 mm Hg yields 96 % sensitivity and 95 % specificity for HFpEF. [@1]
  • Exercise stress echocardiography with Doppler assessment of E/e′ and estimation of pulmonary‑artery pressures, or
  • Invasive exercise hemodynamic testing (supine bicycle) measuring PCWP, pulmonary‑artery pressures, stroke volume, and cardiac output. [@2][@3]
