Treatment of Gastrointestinal Inflammation
Ulcerative Colitis Treatment
- The American Gastroenterological Association recommends initiating treatment for ulcerative colitis based on disease type and severity, with topical and/or oral mesalamine for mild-moderate disease or prednisolone 40 mg daily for moderate-severe disease 1, 2
- For distal disease, the European Crohn's and Colitis Organisation suggests starting with topical mesalamine as first-line therapy, with oral mesalamine or topical corticosteroids added if there is an inadequate response or intolerance 2
- In cases of mild to moderate extensive disease, the American Gastroenterological Association recommends initiating oral mesalamine combined with topical mesalamine, with doses greater than 2.4 g/day for optimal efficacy 2
- For moderate to severe disease, the American College of Gastroenterology suggests starting prednisolone 40 mg daily combined with mesalamine, with a gradual taper over 8 weeks according to severity and patient response 1, 2, 3
Crohn's Disease Treatment
- The American Gastroenterological Association recommends initiating high-dose mesalazine 4 g/daily as sufficient initial therapy for mild ileocolonic disease, with consideration of patient preferences to improve adherence 1, 3
- For moderate to severe disease, the European Crohn's and Colitis Organisation suggests starting prednisolone 40 mg daily, with a gradual taper over 8 weeks, and consideration of alternative induction options such as elemental or polymeric diets 1, 3
Maintenance Therapy
- The American Gastroenterological Association recommends lifelong maintenance therapy with aminosalicylates for all patients, especially those with left-sided or extensive disease, with alternative options including azathioprine or mercaptopurine 4
- The American College of Gastroenterology suggests that all smokers must be strongly advised to stop, as smoking cessation is the most important factor in maintaining remission, and recommends azathioprine or mercaptopurine as effective second-line therapy 6
