Alprazolam and Itraconazole Interaction

Mechanism of Interaction

  • Itraconazole is a potent CYP3A4 inhibitor that dramatically increases alprazolam plasma concentrations by blocking its primary metabolic pathway, as stated by the Infectious Diseases Society of America guidelines 1, 2, 3

Clinical Consequences

  • The Infectious Diseases Society of America guidelines classify azole antifungals as having significant drug-drug interactions with psychiatric and neurotropic medications that may adversely affect patient outcomes, including enhanced benzodiazepine toxicity 1, 2, 3, 4

Management Algorithm

  • Echinocandins, such as caspofungin, micafungin, and anidulafungin, lack significant CYP450 interactions and may be considered as alternative antifungal agents if a patient requires ongoing benzodiazepine therapy, according to Praxis Medical Insights 5

Contraindication of Itraconazole and Benzodiazepines

Clinical Consequences and Management

  • The Infectious Diseases Society of America guidelines classify azole antifungals as having significant drug-drug interactions with benzodiazepines, including midazolam and triazolam, that may adversely affect patient outcomes, with a recommendation to avoid these combinations 6, 7
  • The Infectious Diseases Society of America guidelines state that when itraconazole is prescribed, reasons for deviation from the practice of avoiding contraindicated medications, such as benzodiazepines, should be documented in the medical record 6, 7
