Chemotherapy Management in Patients with Hematologic Abnormalities

Patient Assessment and Monitoring

  • A decline in platelet count to 145,000/μL, representing a 27.5% decrease, indicates a need for close monitoring and potential chemotherapy delay 1
  • Hemoglobin levels below 10 g/dL, such as 9.7 g/dL, require consideration of erythropoiesis-stimulating agents (ESAs) or red blood cell (RBC) transfusion 2
  • The American Society of Clinical Oncology recommends regular complete blood count (CBC) checks, including platelet count and hemoglobin levels, to assess the risk of hematologic toxicity 1

Management of Anemia and Thrombocytopenia

  • The National Comprehensive Cancer Network recommends ESAs for chemotherapy-associated anemia when hemoglobin has decreased to less than 10 g/dL 3
  • RBC transfusion is an option for managing anemia, depending on severity and clinical circumstances 2
  • Consideration of dose reduction of 50% for subsequent chemotherapy cycles if platelets recover but remain borderline 1

Guideline Recommendations

  • The American College of Physicians recommends delaying chemotherapy until platelet count recovers to ≥150,000/μL 1
  • The FDA mandates permanent discontinuation of gemcitabine if hemolytic uremic syndrome (HUS) or severe thrombocytopenia develops 4
