Albendazole Treatment Guidelines for Parasitic Infections

Infection-Specific Treatment Recommendations

  • The Infectious Diseases Society of America (IDSA) recommends a single 400 mg dose of albendazole for Ascariasis (roundworm) treatment 1, 2
  • The IDSA recommends a single 400 mg dose of albendazole for Hookworm treatment, which may be extended to 3 days for complete eradication 1
  • The IDSA recommends a single 400 mg dose of albendazole for Enterobiasis (pinworm) treatment 1, 2
  • The IDSA recommends 15 mg/kg/day of albendazole, divided twice daily for 8-30 days, for Neurocysticercosis treatment, with a maximum dose of 1200 mg/day for parenchymal disease 3
  • The IDSA recommends 400 mg of albendazole twice daily for 3-7 days for Strongyloidiasis treatment, although ivermectin is preferred 1
  • The IDSA recommends 400 mg of albendazole daily for 3 days for Taenia species treatment 2

Essential Safety Monitoring and Precautions

  • The IDSA and American Society of Tropical Medicine and Hygiene (ASTMH) guidelines mandate monitoring for hepatotoxicity and leukopenia for treatment courses longer than 14 days, with liver enzymes and complete blood counts checked at baseline and every 2 weeks during therapy 3
  • The IDSA recommends screening for Strongyloides stercoralis before prolonged corticosteroid use in patients with neurocysticercosis, as steroids can precipitate fatal hyperinfection syndrome 3
  • The IDSA recommends excluding Loa loa infection in children who have traveled to endemic regions before co-administering ivermectin 1, 4

Critical Administration and Treatment Considerations

  • The IDSA recommends administering albendazole with food to enhance absorption, and notes that the tablet may be crushed or chewed and swallowed with water, particularly for pediatric patients 3
  • The IDSA recommends concomitant therapy with corticosteroids and anticonvulsants for neurocysticercosis treatment, and requires a fundoscopic examination prior to treatment initiation to exclude retinal involvement 3
